The nurse reports to and works under the supervision of the RN Home Health Director and the Center Director. The nurse renders all care as ordered by the client’s medical provider, acts as a member of the multi-disciplinary team and liaison between healthcare professionals, completes nursing assessments and reassessments, maintains records to ensure agency and regulatory compliance, reports any changes in orders and/or client condition to the supervising RN and client’s provider, and provides health education to staff, clients, and their families/caregivers.
