Job Summary:
Processes and maintains scheduling of Agency personnel for the Home Health consumer. Scheduling of personnel for the Home Health Care consumer must be consistent with medical, ethical, legal, and regulatory requirements of state, federal and accrediting agencies in compliance with Agency policies. Reports patient information regarding coverage of ordered services to appropriate supervisors and clinical staff within the Agency, tracks and distributes daily visit notes.
Essential Functions:
Assists Case Managers with scheduling HHA staff appropriately to patients in accordance with patient need and staff ability
Keeps records of assignments and all changes made in the course of the patient’s care
Assures all schedules are current and available in the electronic medical record
Complies with all company policies and procedures
Maintain privacy and security practices and prevent privacy or security breaches. If breach occurs, take corrective action
Communicates with all clinical staff as scheduled visits may be subject to change
Assists with new employee orientation, relative to scheduling process and expectations for staff with regards to communication of changes to either staff availability or consumer requests
Assess all daily OMR notes and contacts the appropriate clinical staff member for corrections as needed for billing purposes
Responsible for the distribution of daily OMR clinical documentation into the appropriate patient electronic medical record, notifies the clinical staff member for corrections
Responsible for weekly update and tracking on late documentation
On-call services for staffing and patient calls
Maintains a daily current active patient list
Ensures Insurance authorization is updated before visits are scheduled
Evaluates documentation of the clinical staff for visits made not scheduled, visits scheduled not made
Tracks and faxes clinical missed visits to the physician’s office
Daily distribution of visit notes and notifying clinical staff of corrections

Pay: $17.00 - $19.00 per hour
