JOB TITLE : Adolescent Residential Specialist- Acute High Needs
SUMMARY : Direct residential care provider within an acute high needs group home or facility environment and includes 24 / 7 awake staff.
Provides direct care / support to individuals that include : acquisition, retention or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making, and household chores, eating and the preparation of food, and social and adaptive skills necessary to enable the individual to reside in a community residential setting.
EDUCATION AND EXPERIENCE :
Certification : N / A
Experience : Experience in a high needs group home environment preferred.
Knowledge / Skills : Ability to maintain composure in difficult situations. Ability to follow written and verbal directions.
Verbal / written communication skills.
EQUIPMENT : Computer, fax, copier, motor vehicle, household appliances and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES :
1. Responsible for providing supervision and oversight of residents during his / her scheduled shift. -Staff will evidence no critical incidents resulting from a lack of client supervision
2. Provide oversight and monitoring of resident’s behaviors and activities.
3. Assists individuals with daily living needs in preparing meals, cleaning, social interaction, etc. as needed by each individual.
4. Must successfully complete medication training and deliver medication to individuals as prescribed with appropriate documentation.
Staff will evidence no medication errors in the distribution or logging of client medications
5. Drives a 15-passenger van and maintains a safe driving record.
6. Record / chart pertinent information and observations, resident discussions, plans, progress, skills and abilities. -Required daily documentation will be completed timely and in accordance with contract and / or program requirements
7. Implement individualized plans, collect data and submit to supervisor in a timely manner.
8. Coordinate and supervise residential and community socialization activities as per program guidelines.
9. Maintain the residence in a neat and clean manner.
10. Maintain inventory lists and alert supervisor of any low inventories and special needs.
11. Works various schedules and / or residential programs as assigned.
12. Will participate in the training of other residential staff.
13. Ability to work in a high needs facility, to include graveyard shift.
14. Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed.
Staff will evidence timely and successful completion of all training requirements
15. All other duties as assigned.
