TITLE: Direct Support Program Specialist
SUMMARY: Provides and documents contract/grant requirement services to individuals of program.
EDUCATION AND EXPERIENCE:
Education: Bachelor degree in mental health and/or social service related field preferred.
Licensure: N/A
Certification: Must meet requirements of contract/grant.
Experience: Experience in mental health field and/or intellectual disabilities preferred.
Knowledge/Skills: Verbal/written communication skills. Ability to maintain composure in difficult situations.
EQUIPMENT: Computer, fax, copier and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES:
1. May provide direct services to individuals and plan services with families served by the program.
2. Responds calmly and appropriately in crisis situations, implementing effective therapeutic interventions and strategies.
3. Remains current in knowledge of community agencies and resources, both internally and externally, and how to access resources in order to provide information, referrals and a seamless system of care for individuals served by the program.
4. Coordinates services provided with all facets of the program.
5. Function as program representative within the community.
6. Maintain adequate record keeping.
7. Provide Case Management services to individuals on caseload. Communicates with supervisor regarding concerns/developmental opportunities and quality of services provided by the program.
8. Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed.
9. All other duties as assigned.
