JOB TITLE: Community Living Specialist

SUMMARY:

Provides direct care/support in the individual’s home that includes: acquisition, retention or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making, and household chores, eating and the preparation of food, and social and adaptive skills necessary to enable the individual to reside in a community setting.

Support the person to participate in the community according to their individual choice and preference.

Teach the person decision-making skills and selection of realistic options and possible consequences.

Encourage the person to make informed choices, including decisions regarding his/her services.

Encourage the person to become a responsible citizen in his/her community of choice. Teach the person skills needed to take “as much” responsibility as possible for tasks including personal safety, grooming and cleanliness, household chores, meal preparation/healthy eating habits, money management, medication and health related issues, work/employment skills, community awareness, and other skills necessary for the individual to become fully integrated into his/her community of choice.

The Community Living Specialist will provide monitoring for the health and safety of person and person’s belongings during service provision. Follow procedures to report any suspicion of abuse, neglect, and/or exploitation. Be very knowledgeable of all aspects of the ISP including special programs/plans (behavior, nutrition, safety, etc.) and immediately notify the supervisor of any needed changes to the ISP.

Recommend any needed changes in goals/outcomes and risks (health or otherwise). At all times, treat the individual with the highest level of dignity and respect.

EDUCATION AND EXPERIENCE:

Education: High School Diploma/GED required.

Licensure: N/A Certification: N/A Experience: Experience in a direct care environment preferred.

Knowledge/Skills: Ability to maintain composure in difficult situations. Verbal/written communication skills. 

EQUIPMENT: Computer, fax, copier, motor vehicle, household appliances and any other equipment required to perform the functions of the position.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Responsible for working independently in individual’s home.

2. Responsible for providing supervision to individuals on a continuous basis for duration of scheduled shift.

3. Assists individuals with daily living needs in preparing meals, bathing, cleaning, social interaction, etc. as needed by each individual.

4. Must successfully complete medication training and deliver medication to individuals as prescribed with appropriate documentation.

5. Must be able to safely drive a 12-passenger van and maintain a safe driving record.

6. Record/chart pertinent information and observations, resident discussions, plans, progress, skills and abilities.

7. Implement individualized plans, collect data and submit to supervisor in a timely manner.

8. Coordinate and supervise residential and community socialization activities as selected by individuals.

9. Maintain the residence in a neat and clean manner.

10. Monitor health and safety needs of individuals while on duty.

11. Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed.

12. Attend and participate in HCBS training and adhere to requirements, as applicable.

13. All other duties as assigned.
