FORENSIC PATH - FACC
The Forensic PATH team works with individuals who have been involved with the forensic mental health system and assists them with identifying, locating and accessing resources within their community. This team works with persons in Region 1 of Washington State (Spokane, Stevens, Ferry, Pend Oreille, Lincoln, and Adams counties).
  • Work with individuals who have been identified by the Health Care Authority as eligible for the program
  • Assess needs of individuals and provide case management and assistance in connection to resources
  • Assist with access to employment and educational services
  • Staff are SOAR-trained and assist with Social Security benefits application
  • Utilize Client Support Funds for basic needs or to assist with engagement to resources
The Clinician II is a member of a multi-disciplinary team which includes peers, a CDP, Bachelors and Masters level clinical staff. This team provides outreach and engagement services to individuals who have been involved in the criminal justice system or who have been identified as high utilizers of hospital emergency services, resulting in their placement on the HCA referral list. The FACC Team works in close collaboration with the Outpatient Competency Restoration, Forensic Navigators and the Forensic HARPS programs to locate, engage and build rapport with identified individuals in order to provide them case management services. The FACC Team will not duplicate services offered through health and behavioral health benefits provided under other programs but will leverage services otherwise available to enhance the services available to individuals on the referral list.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Locate individuals identified on the HCA referral list.
  • Create or further develop recovery plans and on-going support to promote progress in care.
  • Provision crisis and hospital diversion services to individuals at high risk.
  • Complete suicide and homicide risk assessments to determine a level of response needed to maintain the safety of the client or others.
  • Assist non-Medicaid individuals to access Medicaid as appropriate.
  • Assist with obtaining SOAR services.
  • Provide evaluation and treatment for patients who have behavioral health concerns.
  • Assist and complete care plans and provide assessments and referrals for outpatient or inpatient treatment.
  • Provide consultation, training, and education to staff regarding behavioral health issues as assigned by supervisor.
  • Facilitate use of/access to crisis hospital diversion resources, including Stabilization and/or E&T admission.
  • Provide 1:1 therapy/group services as appropriate Identify and include the client's support network in care coordination Assist clients in identifying and accessing resources which will meet their needs. Coordinate and collaborate with community resources on behalf of clients. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:
  • Master's degree in psychology, counseling, social work or related field from accredited college or university.
  • Must submit for Agency Affiliated Counselor Registration within 30 days of hire.
PREFERRED:
  • Two years of experience working with psychiatrically impaired populations.
  • Experience in the criminal justice system.
KNOWLEDGE, SKILLS & ABILITIES:
  • Ability to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economical status.
  • Must have reliable transportation, insurance and an active drivers' license and be willing to use personal vehicle in the course of work.
PAY RANGE ( Hourly/ Non-Exempt):*
  • Mental Health Clinician II - No Licensure: $31.84 - $38.79
  • Mental Health Clinician III - Licensure: $33.70 - $41.06
*All pay is based on longevity at Frontier Behavioral Health; thus the starting pay for this position is as follows:
  • Mental Health Clinician II - No Licensure: $31.84 per hour.
  • Mental Health Clinician III - Licensure: $33.70 per hour.
