Frontida Assisted Living Inc. is Hiring a Southeastern Regional Nurse Near Kenosha, WI

Maintain active RN license.

Strong communication skills and attention to detail Must actively demonstrate Frontida’ s culture and exhibit extraordinary care toward Frontida’ s residents. Must be CBRF trained and certified.

  • Current Driver’s License
  • Must pass all applicable background checks and health screens as required by the State of Wisconsin Department of Health Services
  • 5 Years of Senior Living Experience
  • Knowledge of relevant regulations and quality standards.
  • Must be able to read, write and communicate in English.

Knowledge, Skills, and Abilities:

  • Extensive working knowledge of DHS 83.
  • Experience in the assisted living industry.
  • Use of computer for various applications, such as patient database management and Microsoft Outlook. Proficient writing, analytical and problem-solving skills.
  • Knowledge of principles and practices of organization, planning, record management and general administration.
  • Ability to communicate effectively.
  • Ability to operate standard office equipment, including but not limited to, computers, telephone systems, copiers, and facsimile machines.
  • Ability to follow oral and written instructions.


Responsibilities:

Leadership and Professional Conduct

  • Is a positive representative of the company and sets a strong example of professional conduct and appearance for employees and management staff.

Community Management

  • Maintain current knowledge of State Regulations and routinely monitor Community clinical compliance and risk review.
  • Ensure understanding and compliance with all regulations regarding Residents’ rights.
  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home like environment for the residents.
  • Works closely with the Executive Director to counsel the resident and designated agent(s) meeting personal and related health care needs. Notifies physician and/or designated agent(s) of resident status/change as appropriate.
  • Assures a high degree of customer satisfaction at Communities. Promptly investigates complaints and reports findings and appropriate recommendation to the Executive Director. Ensures follow-up communication occurs to complainant and that substantiated complaints are resolved to reasonable satisfaction of complainant.
  • Responds to resident or staff concerns and responds appropriately including investigating, resolving and/or referring them to the Executive Director.
  • Attends service plan conferences, maintains service and treatment records for each resident as required.
  • Acts as an advocate for Residents and maintains communication with designated agents as requested and required.
  • Ensures compliance with all rules and regulations related to resident care (i.e., HIPAA, State Regulations and OSHA, etc.).

Financial Management

  • Communicates clinical supply needs to Executive Directors to maintain the established budget.
  • Conducts Resident scheduled and change of condition assessments on a timely basis and charges in accordance with assessment. Communicates level of care change to the Executive Director.

Program Development and Implementation

  • Utilize nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the resident's care.
  • Participate in communities state surveys as necessary or as directed.
  • Monitor electronic charting compliance to include medication charting, observations, incident and fall reports, care plans, assessments, and behavior support plans.
  • Monitor through weekly communication with communities, clinical risk and assist in development of proactive action resolutions and training opportunities.
  • Participate in QA meetings with Nurses, and Executive Directors as Directed by VP of Clinical Operations.
  • Complete and produce reports as requested.
  • Perform education and training for team members to ensure best practice is being performed for all residents.
  • Perform resident communication with medical providers as appropriate.
  • Ensure a Holistic approach to meet residents’ needs to promote their highest well-being.
  • Coach and mentor Nurses to ensure support and guidance in all areas.
  • Perform performance improvement discussions as needed.
  • Interview qualified candidates for open positions as applicable
  • Perform onboarding of Nurses as needed.
  • Provides and oversees care within Frontida’ s Care philosophies in accordance with policies and company standards.
  • Conducts evaluations, individual service plans, develops service schedules and updates service information as indicates by Resident need. Addresses physical, social, spiritual, and psychological needs of residents.
  • Supervises and assists in provision of all Resident services, including the coordination of those provided by independently contracted ancillary health care providers as requested by Residents and families. Supervises and performs resident services (e.g., health care monitoring, nail care, medication assistance, etc.) based on individual resident service plans.
  • Oversight of Administration of medications and treatments, based on individual resident service plans; observes results and documents same. Run reports to monitor electronic health record compliance.
    • Coordinates delegation of medication administration/assistance and health care tasks per state regulations and Nurse Practice Act.
  • Conducts and/or coordinates various health care “clinics” for residents.
  • Conduct Quarterly Review of Psychotropic Medications
  • Assists with move-in and orientation of new residents and designated resident(s).
  • Coordinates move-out of residents with Executive director, as assigned.
  • Coordinate with interdisciplinary team for continuity of care.
  • Monitor for accurate transcription of physician orders between pharmacy interaction and staff acceptance into medical record.
  • Assists with move-in and orientation of new residents and designated resident(s).
  • Assures proper records are maintained. Documents nurse’s notes and change of condition.
  • Assures infection control procedures including reporting requirements are known/followed by staff.
    • Ensures residents and staff receive Mantoux tests in the most appropriate, cost-effective manner.

Staffing and Retention

  • Oversee and identifies resident and staff training needs. Including standard precautions, Medication Administration, First Aid with Choking Prevention, and other regulatory requirements. Contribute to new team member orientation per Frontida standards.
  • Assist in evaluating performance, provide feedback, assist, coach, and discipline staff, as necessary.
  • Monitor employee morale, provide mentorship and a supportive team environment, and encourage the professional growth of all employees through orientation, training, and ongoing education.
  • Collaborate with risk management on matters requiring legal counsel as appropriate.

Marketing and Census Development

  • Work collaboratively with the Regional Sales Director to execute campaigns for the Community.
  • Ensure clinical, sales and operations are working as a unit in the community to provide quick turn-around of prospect to increase lead to close ratios.
  • Participate in marketing events such as Open Houses, Community Participation Events, and all other Frontida, Inc. events.

Community Relations

  • Develop and maintain positive relationships with State regulators, the communities at large, families, Residents, ombudsmen, and other professionals on behalf of the Community and the Company.

Physical Working Condition Descriptions:

  • Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds, occasionally lift and/or move up to 50 pounds.
  • Requires prolonged sitting, standing, frequent bending, stooping, or stretching.
  • Requires sitting and standing associated with normal office environment.
  • Requires regular use of a computer and company specific software systems.
  • Requires driving and/or traveling throughout region.
