Job Overview

We are searching for a talented and professional Case Manger to maintain and assist homeless Veterans transition into permanent housing. You will represent the temporary emergency shelter at the Paget Center and work with cross functional teams to develop, coordinate and manage Individual Care Plans for each Veteran. Take ownership with cross functional collaboration to ensure accuracy of Veteran’s successful housing plans.

You must be self-motivated, enthusiastic, and community-driven to take on this exciting role. This position requires someone who possesses a positive attitude and readiness to be a team player.

Responsibilities:

· Community Outreach, screening, and intaking Veterans to the Paget Center under the Veteran’s Administration contract guidelines

· Connect Veterans to countywide community partners (i.e. Nation’s Finest, Housing Matters, HUD VASH, VRSI employment specialists, Veteran’s service office, and medical/mental health services

· Create and implement Individual Care Plans for each Veteran (i.e. goals and tasks to complete while at Paget)

· Provide materials and training to assist in Veteran’s housing and employment goals

· Monitor Veteran’s progress and participation toward goals, collect and report outcome to the VA liaison within required timelines

· Document and report all critical incidents/emergencies

· Log all services provided to the Veterans in their respective charts

· Prepare and participate in the quarterly audit by the VA liaison, maintaining 95-100% compliance as mandated by the VA

· Monitor and report changes in Veteran’s physical or mental health status to the staff, House manager and VA liaison

· Initiate preventative measures and create intervention methods for Veteran for any conditions that pose a threat of danger or crisis

· Transport/assist Veteran in scheduling medical appointments, court dates, financial business and interviews with landlords

· Participate in case management with each Veteran on a weekly basis.

· Collaborate with VA liaison regarding care plans, eligibility and any issues that may arise

· Keep clear, accurate and profession records, perform written, verbal and in-person communication with various agencies and vendors

Qualifications


· High School diploma or equivalent

· Ideal for BSW/Human Services major student

· Outstanding written communication and presentation skills

· Experience with public speaking

· Experience with homeless outreach, ideally with homeless Veterans

· Proficient in MS Office with Outlook, Excel & Word. Web browser savvy.

· Maintain a good driving record and be able to pass a background check

· Friendly, enthusiastic, and positive attitude

· Veteran preferred but not required

· Ability to manage multiple tasks at the same time

All Final candidates must be approved by the Department of Veteran’s Affairs.
