FRONT ST MASTER is Hiring an APPCOORD Appointment Coordinator -Drake House Near Monterey, CA

SUMMARY: Arranges and provides transportation to medical, dental, and psychiatric appointments for adults and older adults in a residential care facility. Responsible for overseeing the planning, coordination and management of appointments, documenting appointment outcomes and communicating outcomes and issues to the treatment team regularly. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Must be able to perform each essential job function satisfactorily.

Direct Service and Appointment Coordination

  • In a culturally competent manner will provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
  • Provide residents with counseling and training and as needed assistance with daily living skills and managing psychiatric symptoms.
  • Assist in identifying and accessing medical, educational, prevocational, rehabilitative, social and other community services, specific to residents’ needs, goals, and service plans.
  • Manage the coordination of appointments with a goal on maximizing efficiency while maintaining a high degree of client satisfaction.
  • Maintain positive relationships with other service providers in the community including physicians and medical office personnel.
  • Ensure that medication and other treatment orders are written in adherence to title 22 and company standards.
  • Initiate preventative measures and create intervention methods for clients or conditions that pose a threat of danger or crisis.
  • Inform individuals of their personal rights, responsibilities and Company’s grievance procedures. Ensure services are compatible with residents’ freedom of choice.
  • Transport residents to appointments, outings, etc. Maintain vehicle inspection logs.
  • Provide information to clients, families, other agencies and the community regarding health care issues including prevention, early detection and remedial care.
  • Other duties and responsibilities as assigned.

Teamwork and Collaboration

  • Maintain cooperative and professional working relationships and communications.
  • Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct resident satisfaction surveys.

Reporting and Documentation

  • Mandated to report any “suspected abuse of dependent adults and elders” directly to law enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
  • Monitor residents’ progress and participation toward service plan goals and treatment compliance and collect and report outcome data. Document coordination in progress notes and complete all documentation (i.e. progress notes) within required timelines.
  • Keep clear, accurate, and professional records in resident’s files, staff logs, and other required communications.
  • Meet Title 9 and company guidelines for documentation and attend QIC as needed.
  • Meet individual billing expectations for Units of Service (UOS) and work collaboratively to meet program requirements.

Activities (as assigned)

  • Help organize and conduct group treatment and recovery oriented structural activities.
  • Give capable residents the opportunity to participate in the planning, preparation, conduct, clean-up and critique of the activities.
  • Adhere to monthly schedule of planned activities.

Education and/or Experience

B.A. or B.S. in social work or a related field or two years experience working in the mental health field.

  • Must be at least 18 years of age.
  • If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
  • Cleared by the Department of Justice and FBI prior to commencing work at the facility.
  • TB clearance within the last 6 months and yearly thereafter. Pass a health screening.
  • Maintain a valid First-Aid certification.
  • May be required to serve on company committees.
  • Complete all annual training requirements as mandated by Community Care Licensing Division, Title 22 and company policies.
  • Attend mandated meetings and trainings; fulfill all coursework and training hours required
  • Work in a safe and acceptable manner, following established safety procedures.

This position will be located at Drake House, a 49-bed Residential Care Facility for the Elderly (RCFE) which is just blocks away from Cannery Row and the Monterey Bay Aquarium. Drake house Specializes in providing healthy aging and wellness in a spectacular setting. This is a Full-time position eligible for Health benefits, 401K with match, holiday, Paid Time Off (PTO).
