Fresh Start Therapeutic Services, LLC is a qualified wrap facility and mental health clinic. We offer intensive outpatient mental and behavioral health services to clients who are at risk of homelessness, hospitalization, school expulsion, or cannot fully function in all life domains. We offer psychiatric, life skills, therapy, and case management services.

We are searching for Community Support Service Providers (CSSP) to join our team and assist us in providing life skills training to our clients. Our CSSPs will be the primary point of contact with our clients and will spend 3-6 days a week working with a single client. Travel around the Houston area to client's homes for sessions is mandatory.

Qualifications:

Applicants should have outstanding verbal and written communication skills, including previous experience with medical documentation. Applicants should expect to work 3-6 days a week with a single client and may have a caseload of 1-3 clients. Applicants must be available for orientation, 4 staff meetings Fridays at 10 am, and monthly In Services to continue training and development.

Job Duties:

 Provide skills-building service delivery to clients that identify, enhances capabilities, knowledge, skills, and assets of child, adolescent, LAR, the identified client, and their community. The focus should be on increasing functionality and assets rather than on eliminating deficits.

 Be able to assist in recovery- a process of change through which individuals improve their mental health and wellness, live a self-directed independent life, and strive to reach their full potential.

 Be able to provide site-based intensive services where the location of services provided is usually where the client lives, school, after school, adult daycare, etc.

 Review treatment plan with the client, and the LAR as required specifying the individual’s recovery goals, objectives, and interventions that guide the recovery process. This will be done at the initial session and periodically throughout the course of treatment.

 Update schedule in Theranest calendar DAILY as to not overlap other clients’ or Case Manager schedules.

 Document in Theranest in “Progress Note” section specifically following the Course of the Treatment protocol for documentation and the Treatment Plan. Documentation should be professional using proper verbiage, grammar, and punctuation. Notes are to be entered into Theranest no later than midnight on the day of the service/client meeting. Note corrections should be sent back within 24 hours. See Course of Treatment Handout.

 Document any and all correspondence other than actual session visits with clients and/or LARs in
Theranest under Contact Log in a timely manner.

 Attend Friday staffing meetings and Treatment Plan Reviews with the treatment team after reviewing the treatment plan with the client about what changes are needed. Review new treatment plan with client after any changes are made.

Requirements:

 Must have 2 years providing direct mental health care experience
 Must have an associate in social services or higher education
 Must have reliable transportation
 Must have a valid driver's license
 Must have current car insurance
 Must undergo a background check in accordance with local laws and regulations
 Must submit Tuberculosis Skin Test with "Negative" reading upon offer
 Must submit First Aid/CPR certification upon offer
 Must obtain an ANSA and CANS Certificate upon offer

This is a contract position meaning 1099, with a varying contract length. Full-time, part-time and PRN positions are available. Fresh Start Therapeutic Services, LLC does not offer mileage reimbursement and will only pay hours billed as an independent contractor for hours spent with the client directly or telehealth due to the recent COVID-19 pandemic. Opportunities for advancement are available to those with a Bachelor's degree or higher.

Applicants will be required to submit all orientation paperwork and complete the training process before being assigned to a client.
