Job Description

8th Grade Transition Support Specialist

 Qualifications:

  1. Degree and/or appropriate experience in Social Work, Counseling, Education, or other closely related field.
  2. Experience and/or demonstrated aptitude for working with at-risk juveniles.

Reports to: Designated Building Administrator(s)

Job Goal: Successfully transition students to the 9th grade comprehensive academic setting who are unready for 9th grade for a variety of reasons: attendance, academic, and behavioral. 

Support students and advocate for their emotional, mental and physical wellbeing to promote academic success. Collaborate and provide academic and behavior interventions for identified at risk students.

Performance Responsibilities:

  1. Manage and assist students in an online learning environment.
  2. Coordinate services and educational goals for successful transition to the comprehensive school setting.
  3. Communicate at least weekly with families via phone, video conference, and/or in person.
  4. Work with individual students to devise a Student Success Plan with appropriate goals, objectives, and time lines to help students reach attainable goals.
  5. Engage strategies to help students develop planning and communication skills, coping strategies, study habits, and resilience.
  6. Coordinate activities to help the student reintegrate into family, community, and learning environments.
  7. Connect staff and peer support with students served. 
  8. Coordinate interactions between students and families with agencies that address issues affecting the home/school environment and consult, with permission of the family, to provide wrap-around services as necessary with the family and community agencies.
  9. Track student attendance, academic progress and behavior to help provide resources for the student to attend school.
  10. Maintain written records concerning the work related to the position, such as agency referrals, student program outlines, task log, and similar records to facilitate the position and prepare summaries of tasks completed.
  11. Maintain confidentiality standards as defined by local, state, and federal rules and regulations.
  12. Work with appropriate staff including, school social workers, counselors, administration and other support staff to plan, implement and evaluate programs and services to meet the needs of students assigned.
  13. Perform other duties assigned from time to time by supervisor(s).

Terms of Employment: Salary or hourly pay rate and work year to be established by the Board with consultation and recommendation of the Assistant Superintendent. Designated by Notice of Assignment and funding source.

Evaluation: The designated Building Administrator(s) shall evaluate the performance of this position.

Clearances: Criminal Justice Background Clearance
