Freeport Memorial Hospital is Hiring a TEAM LEADER NURSING - TELEMETRY Near Freeport, IL

Job Summary

Schedule for this position is 3pm-3am - Sundays, Thursdays and Fridays.

Supports initiatives assigned by the Nursing Operations Leaders for Med-Surg, Telemetry, and ICU on the off shifts with a focus on quality and service excellence using evidence-based standards of care. Supports and is an example for a positive work culture focused on excellent communications and practices that encourage team engagement. Serves as a link to all disciplines for patient care to assure desired outcomes, mediates, and problem-solves concerns related to patient, staff and physicians.

The positions main focus will be to support established operational practices as they are assigned including time and attendance monitoring, staff performance evaluations, data collection, quality management, patient satisfaction, and staff performance on a day to day and on-going basis. Will support staff with compliance to policy, procedures, patient care standards and regulatory guidelines. Will guide and support staff with delivery of patient care, patient assignments, staff supervision and problem-solving/mediations as necessary.

Job Responsibilities

  • Demonstrates clinical competency in providing age specific care to patients in designated area of practice.
  • Utilizes evidence-based practice standards in delivery of patient care.
  • Supports and participates in Shared Leadership/Governance and interdisciplinary decision making.
  • Assist with staff compliance initiatives, policies, procedures, patient care standards, and regulatory guidelines.
  • Quality of Care monitoring data collections and reporting. Examples include:
    • Chart audits as assigned
    • Follow-up on Quality reports as assigned. Validations of staff on FHN assigned initiatives (ex. Hourly rounding, bedside report, etc.)
    • Planned education initiatives and just in time education related to clinical, quality, and service skills.
    • Assist and supports all quality projects
    • Assist and supports nursing validations
  • Round on all units, staff, and patients when needed to mitigate any customer concerns.
  • Support unit clinical work such as Rapid Responses as appropriate, admissions, and clinical skills as needed:
    • Complete phlebotomy course and 8 observations
    • Class on ultrasound guided IV starts
    • PICC/Midline training
  • Work closely with NSO regarding any house-wide issues and staffing concerns.
  • Assists with hiring, orientation, training, competency, and evaluating of staff members.
  • Displays competency for the Team Leader role as evidenced in the annual performance appraisal following FHN Workforce Standards.

Requirements

Education: An Associate’s Degree in Nursing is required, a Bachelor’s Degree (BSN) is preferred.

The registered professional nurse must continuously seek opportunities to enhance their own professional nursing practice. Membership in an appropriate professional nursing organization is recommended and previous leadership experience including charge experience, community leadership, group facilitation, committee or project involvement or an active role in the change process is necessary.


Required Experience:
2 – 5 years of experience is preferred.

Licensure/Certification/Registration: The Team Leader must be a registered nurse and must hold an active license to practice nursingin the state of Illinois.

Must be CPR (BLS) certified.

  • Serves patient in areas other than patient care.
