Job Summary

Delivers professional nursing services to participants of community outreach events. This includes screening activity, wellness/health counseling, patient education, and referral information and vaccination administration.

Demonstrates competencies to deliver screening tests and procedures according to laboratory standards of practice. Demonstrate age-specific competencies for outreach activities to effectively establish rapport and communicate with participants.

Provide excellent customer service and interpersonal skills to positively represent FHN in the community. Serve as a liaison between community members and FHN.

Job Responsibilities

Provide effective health counseling and educational materials based on assessment of participant's needs as identified by screen results and verbalized/identified concerns.

Administer health screen tests/activities in accordance with laboratory standards of practice.

Coordinate, set up, and mange health information displays at a variety of venues including community events, schools, churches, and health fairs.

Demonstrate knowledge of FHN's services, providers, and resources to effectively refer participants to FHN.

Administer flu vaccine in accordance with FHN's medication administration policy and procedure.

Demonstrate competency for all outreach activities to perform screening.

Provide program support as requested.

Ensure completion of appropriate forms by participants.

Requirements

Education: Graduate of an accredited nursing program, two to five years related experience and/or training, equivalent combination of education and experience. Ability to function with minimal supervision in screening setting.

Licensure/Certification/Registration: Registered Nurse currently licensed to practice in the state of Illinois. CPR/AED certified.

While performing this job, the employee does not care for patients in the following age groups.

• Neonate: <30 days
• Infant: <1 year
• Early Childhood: 1 year and <5 years
• Late Childhood: 5 years and <13 years
• Serves patients in areas other than patient care.
