Freeport Memorial Hospital is Hiring an OPERATIONS LEADER - HOSPICE Near Freeport, IL

Job Summary

The Operations Leader of Hospice is responsible for managing and facilitating departmental activities to deliver quality care following regulatory/evidence based standards of care. Works collaboratively with the Director of Care Transitions. Priorities include quality and service excellence, which focuses on patient, staff and provider satisfaction. Supervises and guides staff to ensure that PRIDE standards are demonstrated in the delivery of patient care. Serves as a link to all disciplines for patient care to assure desired outcomes and a link to all Hospice programs at FHN. Mediate and problem solve concerns relating to customers, patients, providers and staff. Works collaboratively with FHN team to ensure fiscal responsibility. Operations Leader for Hospice gives the team a broader scope of support and leadership presence to engage the team to provide care excellence to all patients/families throughout their care within FHN and external organizations.

Job Responsibilities

  • Manage daily operations and ensures follow through
  • Responsible for staffing and scheduling staff
  • Provides direction, guidance and supervision of staff while supporting a culture of high performance
  • Assist with developing policies/procedures as needed. Provides direction, and assures compliance with organizational policies/procedures among FHN providers and staff
  • Monitors compliance and responsible for quality improvement initiatives
  • Complete performance appraisals and onboarding process on or before due dates
  • Evaluates the quality and appropriateness of health care provided. Recommends changes that improve quality of care, work efficiency, financial performance or patient/family satisfaction
  • Conducts interviews of potential new staff and consults with Director as needed
  • Oversees the ordering, receipt and approval of supplies, while monitoring expenditures and conserving resources
  • Monitors and communicates patient satisfaction results, while overseeing the implementation of determined action plans with result follow up
  • Responsible for the coordination of department meetings
  • Address patient, provider and staff concerns/grievances, while considering process improvement initiatives. Consults with Director as needed
  • Actively participates on teams and committees throughout FHN as appropriate to support FHN’s mission, goals, and strategic plan
  • Other duties as assigned

Requirements


Education:
Bachelor’s Degree, in nursing recommended

Required Experience: 5 – 8 years
