Freedom Behavioral Hospital of Monroe is Hiring a LPN - Intensive Outpatient Therapy Coordinator Near Monroe, LA

Freedom Behavioral Hospital of Monroe is taking applications for a full-time IOP LPN. This is a unique position that puts the qualified candidate in a clinical setting as well as networking to referral sources within the community.

The IOP LPN provides direct and indirect patient care services that meet the psycho- social, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations. Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomology. 

Develops, discusses and communicates a plan of care for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.

Demonstrates skills and judgment necessary to ensure accurate medication administration.

Maintains up-to- date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.

Promotes environmental and patient safety.

Provide direct clinical services to consumers on an individual, group, and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms and to promote personal growth and development by assisting consumers to adapt to and cope with internal and external stresses.
Provide multiple-stage individual and group treatment in the office to develop a trusting relationship with consumers so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning.

Other duties included but are not limited to: LPN will be responsible for marketing to referral sources in a 50 mile radius within the community. LPN will participate in community events and visit referral sources, to promote the IOP program. Manage the admissions process for new patients, complete physician rounds as needed, manage the patient medications by calling in prescriptions as instructed, monitoring needed labs for drug levels, follow up with family members or patient advocate about the programs and use of medication protocol and monitor for any needed changes, and other duties as assigned.

PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:

Education: Certification from approved Practical Nursing Program

License: Licensed as an LPN to practice in the State of Louisiana.

Experience: One year LPN experience preferred. Previous experience with behavioral health and/or geriatrics strongly preferred.
