Job Summary
Under the direction of a Director, the Practice Transformation Specialist is dedicated to overseeing the development and delivery of end user onboard training, Electronic Health Record policies, competencies, and workflow integration of quality program requirements. Primary responsibilities include oversight of projects vital to practice transformation, practice acquisitions, and new program expansion. This key position will be collaborating with IT analysts, clinicians, and end users in support of system upgrades, data collection needs, and patient centered medical home. Responsible for data driven continuous improvement initiatives regarding quality performance, clinical outcomes, and IT system optimization.

Essential Functions:
Oversight of strategic alignment and utilization of resources:
• Project Specialist to support physician practice transformation and acquisition efforts
• Implements Frederick Health Medical Group (FHMG) centralized best practice standard operating procedures in alignment with diverse initiatives and programs
• Collaborates and supports PCMH transformation team to develop best practice standard work for review
• Specialist in coordination with Frederick Health Medical Group management, IT support and clinical staff systems to ensure process changes and development of future workflows are appropriate and increase efficiency
Education and Training
• Establishes quality measure performance and competency benchmarks
• Ensures sustainable model for new user onboard training
• A high level of understanding of NextGen clinical EMR with clinical user’s experience.
• Leadership of a physician practice super user program on-going development and goal achievement.
Leadership & Management
• Assists with communicating changes in workflow and best practices during system upgrades and program changes
• Maintains development and leadership Frederick Health Medical Group Super User Program to support continuous education and training efforts
• Collaborates with hospital leadership, physicians, clinicians, PI, Quality, end users, vendors and associates to support the development and deployment of electronic applications or new functionality
• Ability to define problems, investigate obstacles, gather relevant information, engage other key stakeholders and provides a workable solution.
Other Duties as assigned
• Supports Director of Patient Analytics and Quality Performance with implementing new business opportunities and strategies for organizational growth and profitability.

Physical Requirements:
