Transitions works in a strengths-based, culturally relevant, and co-occurring friendly manner with participants. The program utilizes many principles of the Transition to Independence Process model that engages youth in their own recovery process towards healthier expressions of their lives. Service delivery is a collaborative process, helping participants to co-create their short- and long-term goals. We provide community-based services, meeting participants in environments where they feel most comfortable. Services are designed to meet TAY where they are, using modalities and interventions that TAY can relate to, including engaging youth in a nonjudgmental, conversational style of communication, and discussing topics that are relevant to TAY. Services place much emphasis on building a strong therapeutic alliance with our participants in order to foster the most effective outcomes.

What You Will DoThe Mental Health Therapistis responsible for providing clinical assessment (i.e., diagnosis, behavioral health assessments, client plan and progress notes) through a trauma-informed lens, therapeutic based services, and case management to a diverse population of at-risk youth, young adults, and families.

Shifts Available 
Full Time – afternoon and evenings required. Work remotely with clinical services provided in the community, office and telehealth settings.

Salary RangeIn accordance with California law, the expected salary range for this California position is between $33.03 and $53.80 per hour. The actual compensation will be determined based on experience and other factors permitted by law.

What You Bring to The Table (requirements):

1.M.A. Degree in Social Work or Counseling from an accreditedcollegeor university.

2.Registeredassociate (AMFT or ACSW, or APCCwithrelevantexperiencewithtargetpopulation)withtheBoardofBehavioralSciences.Someprogramsmayconsidercandidateswhoareabletoobtainregisteredstatuswithinfirst six (6)monthsafterhire.(If working in a program licensed by Community Care Licensing must meet minimum training and experience requirements - for more information please see http://www.cdss.ca.gov/cdssweb/entres/pdf/CCR/STRTP_ILS.pdf )

3.Demonstratedknowledgeintheareasofdiagnosticassessment,individual/family/groupbehavioraltreatmentmethodsfortargetpopulationofprogram.

4.Demonstrated knowledge of risk assessment, including danger to self or others, and effective community-based interventions.

5.Demonstratedculturalresponsiveness in workingwithdiversefamilies andcommunities.

6.Cleandriving record:currentCADriver’sLicenseorabilitytoobtainwithin10daysafter hired.

7.Musthaveaccesstodependabletransportation;personalcarinsurancemeetingminimumliabilitycoveragerequiredifpositionrequiresdrivingforworkortransportingparticipants.

8.Basiccomputerandkeyboardingcompetencewiththeabilitytouse MicrosoftOffice Suite and/orcompetencywith electronic clinical documentation systems.

Preferred

1.One (1)yeargraduateschool or post graduate experiencewithprogramtargetpopulation(Example: intellectually disabled, homeless, teens or young adults, seriously emotionally disturbed).

2.Appliesastrength-based,culturallyresponsive,trauma-informed, and flexibleapproachtosupervision, participant and communityservice, in order to support participant recovery, health, and wellness.

3.Exhibitsstrongbehavioral,crisismanagementandfamily/individual/grouptherapyskills.

4.Fluencyin prevailinglanguage(s)of thecommunity.
