Position Summary:

To carry and follow instructions regarding direct client/patient care according to the written plan of care authorized by either the Agency’s Registered Nurse or Primary Physician.

The Home Health Aide will also provide assistance to the client with personal hygiene, housekeeping and other related supportive tasks for the client within his/her home.

Reports to: President & RN/Case Manager

Qualifications: Possess valid C.H.H.A./PCA license

Possess communication/ people skills

Responsibilities & Accountabilities:

• As per care plan, take and record vital signs including temperature, pulse, respiration, reporting all significant changes to the Agency.
• Assist and/or remind clients in administering medication, following recommendations supplied by the Primary Physician.
• Record all patient information on the activity sheet of care accurately and in a timely manner.
• Assist the client in all areas of Activities of Daily Living (ADL’s) including, bathing, grooming, oral cleanliness, toileting (incontinence), and dressing.
• Assist the client with safe transfers from bed to chair or wheelchair and to maneuver using cane or walker.
• Prepare balanced meals as per Dietician or RN/Case Manager instructions, assisting with feeding if necessary.
• Carry out necessary housekeeping duties including , dusting, vacuuming, bed making, dish washing within kitchen, bedroom, bathroom and/ or living area.
• Accompany clients to medical visits, e.g.., hospital or Physician Office.

Note: Employee must never use personal transportation for these visits

• Complete errands to grocery stores etc. as requested by client, ensuring that client is left in a safe environment.
• Carry out personal laundry functions, including ironing on request.
• Ensure regular “turning "of “bed bound†clients to prevent Decubiti (bed sores)
• Demonstrate appropriate skin care techniques.
• Assist in range of motion (ROM) exercises as per Therapist or RN recommendations.
• Adhere to universal precautions, aseptic techniques and infection control guidelines.
• Demonstrate the ability to communicate effectively with the Agency, R.N., Physician or other individuals associated with clients care.
• Adhere to policies regarding confidentiality while respecting the rights, privacy and property of the client at all times.
• Ability to utilize telephone time and attendance system
