An employee in this class performs highly responsible work that utilizes specialized social work and case management skills to assess and identify the needs of justice-involved individuals, in an effort to reduce the number of people placed in detention and identify resources for individuals prior to incarceration, and post-release. The employee will be a member of the SMART Program (Substance Use/Mental Health Advocacy Response Team) and work with Law Enforcement Agencies along with other community agencies to connect individuals with behavioral health needs including mental health, substance use, and intellectual developmental disabilities to available services and resources in their community. The Co-Responder Outreach social worker will respond with officers to calls involving behavioral health issues when they occur and/or provide outreach to individuals after a behavioral health crisis. This will involve coordination with the Mobile Crisis Management Services Provider who may initially respond to the behavioral health crisis. The social worker will perform outreach to individuals who are at risk of entering or who have entered the criminal justice system as well as those served by community agencies such as mental health/substance use providers, EMS and DSS. The Social Worker will assess the social determinants of health needs along with completing a case management assessment, develop a service plan, and provide time limited case management to address the identified social determinant of health needs. Work is performed under the direct supervision of the SMART Program Director and is evaluated through observation, review of reports, and discussion of case management.
A graduate of a college or university with a Master’s degree in a human service field and has one year of full time, pre- or post-graduate degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has one year of full time, pre- or post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor’s degree in a human service-related field and has two years of full time, pre- or post-bachelor’s degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has two years of full time, pre- or post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor’s degree in a field other than human services and has four years of full time, pre- or post-bachelor’s degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has four years of full time, pre- or post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse
counseling.
