FRANKLIN COUNTY GOVERNMENT is Hiring a Social Worker III Smart Program Near Louisburg, NC

General Statement of Duties

An employee in this class performs highly responsible work that utilizes specialized social work and case management skills to assess and identify the needs of justice-involved individuals, in an effort to reduce the number of people placed in detention and identify resources for individuals prior to incarceration, and post-release. The employee will be a member of the SMART Program (Substance Use/Mental Health Advocacy Response Team) and work with Law Enforcement Agencies along with other community agencies to connect individuals with behavioral health needs including mental health, substance use, and intellectual developmental disabilities to available services and resources in their community. The Co-Responder Outreach social worker will respond with officers to calls involving behavioral health issues when they occur and/or provide outreach to individuals after a behavioral health crisis. This will involve coordination with the Mobile Crisis Management Services Provider who may initially respond to the behavioral health crisis. The social worker will perform outreach to individuals who are at risk of entering or who have entered the criminal justice system as well as those served by community agencies such as mental health/substance use providers, EMS and DSS. The Social Worker will assess the social determinants of health needs along with completing a case management assessment, develop a service plan, and provide time limited case management to address the identified social determinant of health needs. Work is performed under the direct supervision of the SMART Program Director and is evaluated through observation, review of reports, and discussion of case management.

Duties and Responsibilities/Knowledge, Skills, Abilities

Working knowledge of mental health and substance use disorder processes, including psychological and physical withdrawal.
Ability to recognize co-occurring disorders and make appropriate referrals for continuity of care.
Thorough knowledge of community resources for mental health and substance use disorders.
Ability to work well with other professionals to coordinate care with public and private entities.
Ability to maintain detailed records for the purpose of reporting requirements of the National Opioid Settlement funds.
Ability to communicate effectively and identify with justice-involved people suffering from substance use disorder while distributing information and education to individuals and groups.
Proficient with Microsoft Office tools
Possess strong interpersonal skills with a commitment to providing respectful, competent communication, including awareness and acceptance of cultural and professional differences.
Complete case management assessments to determine needs and thereafter complete a service plan
Assist individuals served with managing symptoms by identifying and addressing barriers related to appointment and medication compliance
Arrange or provide transportation to appointments if there are barriers to transportation.
Assist with completing applications to services/resources if necessary.
Linkage to other needed services and resources to improve areas such as behavioral health, physical health, employment, and housing.
Provide education to the individual, and family as needed, about the ongoing need for outpatient services, medication management, interaction of substance abuse and medications, and overall wellness
Initiate coordination of care amongst all stakeholders, provide updates, and facilitate necessary meetings to improve the individual’s care.
Teach the individual how to be an active participant in services to promote holistic health care and positive outcomes
Organize care activities and sharing information among all the participants concerned with the individual’s care
Make sure the individual’s needs and preferences are known ahead of time and communicated to involved stakeholders.
Educating and informing the individual about their treatment so they can be involved.
Reviewing questions or ideas with the individual served that should be discussed with the provider
Teaching the individual served strategies to use as reminders (setting timer/alarm on electronic device, putting up reminder notes, etc.) for scheduled appointments/events.
Perform various administrative functions related to the records and document service hours.
Complete all administrative requirements in timely and efficient manner.
Adhere to all clinical policies and operating procedures.
Sign all treatment record notes as required and in a timely and efficient manner.
Ensure compliance with uniform documentation and charting guidelines.
Participate in case staffing and other relevant meetings.
Collaborate with physicians, counselors, law enforcement, magistrates, and community resources to provide complex wrap-around services for difficult cases.
Liaison with the Sheriff Patrol Division to divert people struggling with mental health and substance use disorders from detention.
Serve as a technical resource for mental health and substance use evaluation and treatment.
Maintain detailed records for the purpose of reporting requirements of the National Opioid Settlement funds.
Provides a variety of mental/behavioral health services within their scope of practice, including but not limited to; screening, assessment, referral, brief intervention/treatment, consultations, care coordination/management, education, advocacy, and other related outpatient services while maintaining patient confidentiality/ HIPAA.
Other duties as required.
Additional Job Duties
  • Must be available for evening, holiday and weekend work as required.

Desirable Education and Experience

A graduate of a college or university with a Master’s degree in a human service field and has one year of full time, pre- or post-graduate degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has one year of full time, pre- or post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor’s degree in a human service-related field and has two years of full time, pre- or post-bachelor’s degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has two years of full time, pre- or post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor’s degree in a field other than human services and has four years of full time, pre- or post-bachelor’s degree accumulated mental health, I/DD and substance use experience with the population served, or a substance abuse professional who has four years of full time, pre- or post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse
counseling.
