Four Seasons Healthcare Solutions is Hiring an Intake Director Near Brooklyn, NY

The Four Seasons CHHA is seeking an Intake Director to join their team! This position is responsible for the initial screening of referrals, admission planning and initial service planning for patients found eligible for the Certified Home Health Agency. The Intake Director will participate in program marketing to clients, their family members and community organizations, may provide caseload coverage as needed (as part of the Intake team coverage). This position requires travel in the Brooklyn, Bronx and Queens with occasional travel to other regions.

This position reports to the Administrator.

The Four Seasons Nursing and Rehab -CHHA is seeking an Intake Director to join their team! This position is responsible for the initial screening of referrals, admission planning and initial service planning for patients found eligible for the Certified Home Health Agency. In addition, the Intake Director will participate in program marketing to clients, their family members and community organizations. May provide caseload coverage as needed (as part of the Intake team coverage) This position requires travel in the Brooklyn, Bronx and Queens with occasional travel to other regions.

 This position reports into the Administrator.

DUTIES AND RESPONSIBILITIES:

*1. Contacts referred individual and arranges screening visit.

a. Calls referral source when necessary to elicit further information.

b. Contacts referred individuals/caregiver within 2 business days to arrange screening

visit/encounter

*2. Assesses potential patients for program eligibility.

a. Will meet with patients in subacute rehab (SAR) and review all medical information pertinent to a safe discharge to home in addition to hospitalized patients referred to home care prior to their discharge to the community

b. Will verify insurance information and eligibility in addition to obtaining authorizations

c. Will ensure that Managed Care authorizations are in place prior to the referral to home care (This is dependent on the type of insurance).

d. Will document a summary of the patient’s medical status (wound care, insulin administration/education and/or medication management, etc.), ability to perform activities of daily living and mobility status via rehab notes, current needs, the ability of the agency to meet these needs and any other psychosocial factors that may be relevant to the treatment plan.

e. Will attend Care of Transitions and Discharge Planning meetings and collaborates with discharge planners, social workers and members of the clinical team to determine that the client’s home situation is conducive to the care needed.

f. Will evaluate whether safety of agency personnel can be maintained in transit to and while providing services in the patient’s home.

g. Checks for documentation that SAR staff has instructed patient and or caregiver prior to discharge of medically necessary teaching i.e., insulin administration, wound care and consults with MD at SAR to get order to use normal saline solution wet to dry dressing until wound supplies are delivered.

f. Enters all screening information into the EMR, including all demographic information, information regarding patients housing, insurance and social supports, medical diagnosis, recent course of stay at an institution, medications, proposed plan of care and source of insurance coverage.

*3. Updates referral source post-visit/encounter.

a. Provides feedback (to the referral source on the next business day post assessment) after assessment visit

b. Assists in developing alternate plan options medically and/or financially ineligible referrals.

*4. Contacts potential patient/family/significant other and provides the initial introduction to home care and the potential services provided by TNJH CHHA. The nurse will provide clear expectations to the patient and/or caregivers about services that will be provided in the home. If patient has a Managed Care or commercial insurance that requires authorization, the nurse will thoroughly explain the services that will be provided in the home based on their specific coverage

*5. Contacts patients’ facility physician to review plan of treatment, physician role and program procedures for continuity of client services.

a. If patient is appropriate, PCP is contacted prior to discharge to confirm patient is under PCP care and PCP is willing to sign home care. If patient does not have a PCP, patient must agree to visiting MD until PCP can be established.

*6. Assists prospective patients in the admission process and Medicaid eligibility where applicable.

a. Alerts the CHHA clinical team to make referrals for medical and social services for patients via Heads Up email to all liaisons, all RN supervisors, rehab supervisor/ Vendor and to the CHHA Accounting Clerk, on Friday or Monday and uses the email chain to get SOC date from team liaison. Heads Up email will include pt name address, projected services, and insurance

*7. Meets monthly with the Administrator/ DPS of CHHA to report on status of outreach and marketing activities.

a. Collects and maintains reports on intake data.

b. Provides an intake report to management team on a weekly basis.

c. Documents and records all complaints

*8. Collaborates with CHHA supervisor/ Case Manager/DPS and assigned CHHA RN, once patient accepted home care services in establishing initial plan of care and start of service date.

*9. Submits completed pre- admission documents to office support staff when needed.

 Completes necessary and appropriate paperwork including:

a.Home Visit Request Form-Apex Laboratory, Inc –Form for Blood Work done at home-if needed

b.Face to Face Encounter Attestation Facility Referrals-Medicare/Medicaid

c.Contract Regarding Caregiver Agreement Form if needed

d.Transfer Request for Four Seasons Nursing and Rehab Certified Home Health Agency if needed

e. Patient Homecare Information Form-for all patients being discharged

*10. Demonstrates a high degree of commitment to customer service and quality clinical practice.

*11. By choice, the CHHA Intake RN may volunteer to participate in 24 hour on-call coverage for patients with other nurse coordinators. If he/she does participate, he/she will:

a. Assumes on call coverage for patients as scheduled. This 24 hour on-call coverage consists of a 1 week in duration.

b. Carries a cell phone. Will be available and respond to all calls in a timely manner.

c. Call all designated patients while on-call.

d. Documents all correspondences related to on-call activities in the E.H.R./Netsmart

*12. Participates in Four Seasons Nursing and Rehab -CHHA community outreach programs as developed by Home Health Care Agency and / or Department of Public Affairs and Marketing

13. Attends appropriate meetings, case conferences, in-services etc.

a. Attends and participates at all mandatory meetings and in-services.

b. Participates in performance improvement and utilization review committees as designated.

c. Assists in orientation of new home care clinical staff.

*14. Participates in other CHHA nurse duties as may become necessary for the Certified Home Health Agency to maintain patient continuity of care.

 *15. Performs other related duties as required.

BSN Preferred

NY state RN Valid License

Experience intake or CHHA Required as an RN
