Position Details

What you will do?

As the Care Manager you will be responsible for assuring the success of children involved in Four Oaks’ programs. This position analyzes issues affecting youth, and develops and oversees execution of a client/family plan that delivers Trauma-Informed Care. The Care Manager also ensures effective Discharge Planning for a successful transition to ongoing care in order to ensure Four Oaks identifies and mitigates potential risks.

• Engages family and youth to build relationships, trust and a working partnership necessary for the long-term success of clients.
• Independently utilizes professional judgement to pinpoint goals, increasing the skills necessary to reduce identified impairments and build success.
• Determines supports and services (internal and external to Four Oaks) to fortify the achievement of critical successful outcomes.
• Develops a Treatment Plan within 14 days for PMIC or 30 days admission for Group Care that establishes clear, measurable and time-limited outcomes.
• Actively participates in Discharge Planning to ensure a smooth transition.
• Partners with Direct Care Staff to deliver cost-effective skill development and supportive services required to achieve planned outcomes.
• Selects necessary components of care within identified agency programs/services and identifies when program care approaches need for review/changes.
• Documents progress and shares necessary information with care team members in order to ensure all providers understand and align services to accomplish critical goals.
• Assures staff is delivering necessary and effective services, and reviews progress against treatment plan goals.
• Provides feedback to service staff and direct adjustment to developed service plans to assure intended outcomes.

Qualifications & Program Information

You need a Bachelor’s degree in a social related field and at least 2 years of prior experience.
