Foundations For Life is a residential group care facility supporting the needs of youth with developmental and behavioral disabilities. Foundations For Life On-Site Program Managers work closely with the youth in care, Direct Care Staff, and Administrative personnel (Executive Director, Director of Operations, Licensed Mental Health Clinician, Human Resources, Etc.). The On-site Program Manager also serves as the Foundations For life point of contact between youth in care and their Child/ Family Team (Social Workers, GAL, CASA, Attorneys, WISE Team, schools and various other service providers).

Essential Functions:

• Maintain a positive therapeutic environment within program
• Coordinate and oversee the facility's day to day programming
• Assess the daily living needs of the youth and delegate appropriate tasks to supervisory and direct care staff
• Ensure all medical, dental, vision, psychiatric, or other therapeutic appointments are scheduled and attended as designated by treatment plans/ goals
• Ensure medications orders are placed, inventoried, appropriately stored and both MAR / Documentation and administration policies are adhered to.
• Contribute to Client Treatment Plans, identifying goals, objectives and target behaviors
• Ensure compliance with each child's or youth's treatment and care plan
• Ensure house program, facility, documentation, and client files are maintained to licensing standards
• Monitor staff development and training
• Plan and coordinated monthly outings and enrichment activities

Requirements:

• Master's Degree in social service or a closely related field; AND one (1) year of documented experience working with children or youth.

OR

• Bachelor's Degree in social service or a closely related field; AND three (3) years of documented experience working with children or youth.

OR

• Associate's Degree in social services or a closely related field; AND four (4) years documented experience working with children or youth.
