Foundations was founded by a Speech Language Pathologist and is an experienced resource, utilized by Arizona school districts, making available highly trained, qualified, vetted, and certified therapy providers. Employing evidence-based practices, our providers meet the student’s needs, as well as assist districts to maintain compliance with requirements. We aspire to obtain a dedicated and energetic Speech Language Pathologist Assistant so that we can continue to help students grow and learn in the educational setting. Duties and Responsibilities include:

• Provide direct therapy assistance to students identified by the supervising Speech Language Pathologist (SLP) following written therapy plans, IEPs or protocols developed by the SLP (See Arizona Department of Health Service guidelines for SLPAs.)
• Document student progress toward meeting established objectives as stated in the therapy plan, IEP or protocol and report this information to the SLP.
• Assist with informal documentation as directed by the SLP.
• Prepare and process record keeping and materials necessary for documentation to convey information to appropriate parties.

Requirements:

• Hold a valid SLPA license in the state of Arizona
• Hold a current AZ Fingerprint Clearance Card
• Maintain Continuing Education requirements, as per AzDHS (http://azdhs.gov/licensing/special/index.php#speech-hearing-continuing-education)
