Who we're looking for:

A passionate, dedicated, sharp mental health provider who wants to be part of a growing, collaborative team. As an Integrated Care Clinician, you will be providing clinical services across the lifespan in an integrated healthcare setting that provides primary care. This is an in-person, unique opportunity for a clinician who thrives working independently, and be an integral part of a cutting-edge healthcare model.

What you'll be doing:

  • Maintain a caseload mixed with complex Medical / Behavioral Health cases and Forge general population.
  • Participate as a member of a multidisciplinary team assigned to the client and participate in case management meetings.
  • Provide behavioral health consultation to the primary care providers, while assisting clients in meeting medical and behavioral health treatment compliance.
  • Completion of full assessments for clients, as well as assessing for appropriate level of care and formulating diagnosis in accordance with the Diagnostic Statistical Manual (DSM).
  • Obtain and review records relevant to the current treatment episode, and contact to relevant referral sources, family or outside systems as needed.
  • Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable in conjunction with a multidisciplinary team.
  • Coordinate with discharge planner regarding continuing care and recommendations for discharge.
  • Documentation of all counseling services in clinical records.
  • Maintain all necessary logs required for administrative tracking.
  • Complete relevant state-required work for clients on your caseload.
  • Communicate with family members, attorneys, EAPS, doctors or other outside resources, as needed, within provision of consent to release documents, and document all communications.
  • Initiate releases, contracts for safety or any other necessary documents as needed.
  • Provide crisis intervention, triage clinical emergencies, including suicide and/or danger to others, and other functions as needed.
  • Works with clinical leadership or designee to develop, monitor, and analyze clinical and program performance and outcomes and develops and monitors quality improvement program goals.
  • Perform other duties as assigned.

What you'll need:

  • Valid, unrestricted license in New Jersey as a LPC, LCSW, PsyD, or PhD
  • Masters or Doctorate Degree in Social Work or a similar field.
  • Demonstrated knowledge working with multidisciplinary teams.
  • Demonstrated knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family.
  • Demonstrated knowledge of the use of psychotropic medications.
  • Experience providing consultation to Medical Providers.
