Seeking a Nurse Practitioner to join our team or providers giving care to nursing home residents. You will see a daily patient caseload of about 20-25 Patients a day on different levels of acuity. You will be a team player, working with the other providers on staff . This is a rewarding opportunity to make a positive impact on the lives of patients in need.

You will Conduct physical examinations and obtain medical histories, Diagnose and treat acute and chronic illnesses, Prescribe medications and develop treatment plans, Collaborate with physicians and other healthcare professionals to coordinate patient care & Maintain accurate medical documentation in compliance with HIPAA regulations

REQUIREMENTS:
- Valid Nurse Practitioner license in NY
- Board Certification
- Go getter attitude, and good problem solving skills
- Ability to work independently and collaboratively in a team environment
- Exceptional interpersonal and communication skills
