Fond du Lac County is Hiring a Children's Long Term Support Service Coordinator Near Fond Du Lac, WI

Under general direction determines eligibility, authorizes and coordinates services to assist families who qualify for Children's Long Term Support Waiver (CLTS) or Children's Community Options Program (CCOP).The following duties are normal for this position. These are not to be construed as exclusive or all -inclusive. Other duties may be required and assigned. This position will:
* Assists families in the process of applying for Children’s Long Term Support Services as appropriate.
* Performs intake assessments and interviews collateral sources to determine if individual is eligible for services. Provide families with information, support, and opportunities as needed to enable them to make informed decisions.
* Authorizes and coordinates services to assist families to care for their children in their home. Provides and coordinates individualized comprehensive services to assist families in caring for their children in their homes.
* Understand the Service Delivery System, value natural supports, acknowledge the limitations of budgets, funding, costs, and choices; ensuring resources are effective, efficient, and have value to the participant.
* Coordinate and facilitate the development of Individual Service Plans that meet the needs of families and maintain measures of progress in meeting outcomes. This includes on-going monitoring and targeted case management practice. Creates a budget and manages funding allocated to each case. Reviews invoices and service codes and authorizes invoices for payment.
* Maximize and support the family’s ability to be self-sufficient while accessing community resources relevant to their needs.
* Provide Support and Service Coordination Services efficiently and effectively which reflects the philosophy of self-determination of the Children’s Long-Term Support Program and Children’s Community Options Program. Document all support and service coordination activities and complete paperwork as required.
* Work to build and maintain positive community partnerships and participate in meetings and training opportunities to enhance skills.
* Be available and responsive to families and team members by maintaining flexibility, confidentiality and punctuality. This may include working unstructured hours, including evening hours.Desirable Knowledge and Abilities:
* Person must possess a Bachelor’s degree in Social Work or related Human Services field and one year of experience working with families who have children with disabilities.
* The ability to adjust/cope with a constant caseload demand, efficiently prioritizing caseload and service needs to ensure priority services are delivered.
* The demonstrated history to skillfully plan and organize work to achieve the most effective delivery system.
* The ability to communicate effectively and professionally, both verbally and in writing.
* The ability to use a county issued computer for Microsoft programs, internet, and program specific database management systems.
* General math knowledge to monitor service logs, expense forms, invoices, and authorizations.
* The ability to facilitate and participate in meetings with a variety of people in an array of settings.
* A demonstrated ability to be a self-starter.
* Flexible in meeting the varied needs of families and workload.

Special Requirements:
* Must possess a valid driver’s license and have access to a reliable, insured vehicle in order to provide services in a variety of settings.
* Ability to work unstructured hours, including evening hours.
* The ability to perform the physical requirements of the job necessary for this target group.
