We at Folsom Lake Primary Care, an Internal Medicine clinic in the beautiful city of Folsom, are looking for a dedicated, motivated, energetic Nurse Practitioner with strong communication and interpersonal skills to be part of our team. Currently the position is part-time or full time with competitive salary and bonus. The Nurse Practitioner helps with all aspects of patient care, including diagnoses, treatments, and consultations. He/she will also perform the important task of educating patients about preventative care and prescribed treatments. They may also conduct physicals, order tests and may serve as a patient's primary healthcare provider. Candidate must feel proficient with female wellness exam including pelvic exam and pap smears.
RESPONSIBILITIES
· Conducts physical examinations and interprets medical histories.
· Orders and interprets diagnostic tests.
· Performs diagnostic procedures.
· Counsels and educates patients on health and wellness and disease prevention.
· Conducts assessments of a patient’s health status.
· Develops appropriate plan of care and treatment plans
· Consults with supervising physician or other clinicians as dictated for patient assessment and findings
· Reviews and addresses staff and patient messages and requests daily
· Reviews medical record for follow-up or ordering of diagnostic studies under established protocols.
· Establishes and maintains communication with other healthcare providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care.
· Establishes effective working relationships with members of the health care team, patients, and families.
· Acknowledges staff and patient rights and cultural and ethical beliefs.
· Delegates appropriately and coordinates duties/activities of health care team members.
· Evaluates effectiveness of care given by team members.
· Demonstrates and applies knowledge of safety principles as identified by the institution.
· Performs efficiently in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
· Performs other related duties as necessary.
· Follows up and evaluates the patient’s status.
· Promotes optimal health for each patient.
· Provides competent care in each interaction.
· Promotes a safe environment and ensures that HIPAA guidelines are always followed.
EDUCATION & EXPERIENCE
· Graduate of an accredited School of Nursing Program.
· Graduate of an accredited Nurse Practitioner Program.
· Current Advanced Practice Registered Nurse (APRN) Licensure.
· Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
· Strong computer skills, including Word, Excel, and PowerPoint.
· Experience with an Electronic Medical Record, specifically, EPIC EMR.
· A 2 years’ experience in a Nurse Practitioner role preferred

SALARY AND BENEFITS
· Competitive salary range depending on experience
· A three-month probation period with evaluation at the end of three months
· No weekend schedules
