POSITION SUMMARY:

To provide treatments and nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices, regulations and directed by the Director of Nursing. To lead or direct licensed and non-professional staff in the delivery of direct Resident care and support functions.

QUALIFICATIONS:

  • Must have a current LVN/RN license.
  • CPR certification required.
  • Ability to understand, remember and carry out verbal or written instructions in English.
  • Ability to complete assignments in a timely manner.
  • Ability to work flexibly and positively with intermittent interruptions.
  • Must have at least one (1) year of experience in either an acute medical/nursing center or a long-term care center.

ESSENTIAL FUNCTIONS:

  • Must have working knowledge of the basic principles of infection control and wound care.
  • Must have demonstrated competence in providing wound care that is sterile and/or aseptic.
  • Must have knowledge and understanding of the special needs of the geriatric client and the geriatric client's skin.
  • Must be free of any communicable disease and must maintain hands with clean, non-jagged nails which are of appropriate length to assure residents are cared for in a risk free manner.
  • Provides wound care (bandages, cleaning, applying medication) and assessing for appropriateness of bandage integrity during time between dressing changes.
  • Assuring availability of treatment supplies and topical medications (as prescribed by the physician) and procuring supplies to assure adequate inventory or Treatment Cart is readily available at all times.
  • Providing gastrostomy tube feeding care, including needed bandage changes.
  • Providing colostomy and urostomy care and dressings as directed by the physician.
  • Provide weekly documentation.
  • Complies with facility guidelines for documentation and physician's orders.
  • Relates well to patients, families and other team members.
  • Interacts well with therapists and implements individual programs as developed.
  • Maintains equipment and supplies for treatment.
  • Maintains consistent patient care schedule and is efficient in use of time. Coordinates restorative treatments with other facility departments.
  • Able to perform duties with a minimum of supervision.
  • Identifies potential problems/issues and reports to supervisor in a timely manner.
  • Maintains a professional approach with confidentiality.
  • Is supportive of facility goals and objectives.
  • Cooperates and maintains good rapport with nursing staff, medical staff, other departments, and visitors.
  • Obtains and documents nursing history from the patient and/or significant other that is comprehensive of present condition and prior problems.
  • Performs basic assessment of the patient, indicating adequate knowledge of anatomy, physiology, and pathology.
  • Records vital signs, notes changes, and pursues more-specific investigation as needed.
  • Assesses for and notifies physician and other appropriate parties of changes in condition.
  • Uses assessment information to develop a care plan before the end of duty time that communicates enough information for incoming personnel to adequately care for the patient.
  • Uses care plans as a basis for providing safe and therapeutic care to patients.
  • Consistently follows established standards, policies and procedures in providing nursing care.
  • Accepts accountability for clinical care of assigned patients, including supervision of nonprofessional personnel.
  • Communicates with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan.
  • Performs competently those technical skills considered basic to the type of care given on that unit.
  • Responds appropriately in urgent and/or emergency situations.
  • Supports standards of nursing care through adherence to existing policies and procedures.
  • Seeks guidance when encountering nursing procedures and responsibilities that have not been mastered.
  • Promotes positive public relations with patients, residents, family members and guests
  • Uses tactful, appropriate communications in sensitive and emotional situations.
  • Other duties as assigned.
  • Use electronic medical record and charting system.

PHYSICAL REQUIREMENTS:

Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching and lifting up to 50 pounds with or without reasonable accommodations
