FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.

This is a direct hire, permanent, full-time position in the Hudson Valley Region.

Schedule: Monday-Friday, 9am-5pm

The Nurse Case Manager will coordinate and provide nursing care to individuals with intellectual and developmental disabilities (I/DD) enrolled in the day habilitation and residential facilities.

Responsibilities

• Coordinate the delivery of health care services for adults with disabilities residing in residential programs;
• Be responsible for performing nursing assessments and diagnoses;
• Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.

Requirements

• NYS RN License, BSN preferred.
• Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
• Excellent oral and written communication skills.
• Able to work flexible hours and/or days.

Pay: $88,000.00 - $92,000.00 per year
