FLACRA is Hiring a HCBS UNLICENSED BEHAVIORAL HEALTH PROVIDER Near Clifton Springs, NY

Major Responsibilities :

The HCBS Unlicensed Behavioral Health Provider is an active member in the FLACRA team and provides services identified and approved through HARP Care Management and Managed Care Organization (MCO) to clients with who meet eligibility criteria for HCBS services.

Under the supervision of the Licensed Program Director the HCBS Unlicensed Behavioral Health Provider will function to provide the following service types:

· Psychosocial Rehabilitation (PSR)

· Habilitation (HAB)

· Family Support and Training (FST)

· Education Support Services (ESS)

· Empowerment Service – Peer Supports (EMP)

· Pre-Vocational Services (PVOC)

Each service type is defined in the New York State: Health and Recovery Plan (HARP) Adult Behavioral Health Home and Community Based Services (BH HCBS) Provider Manual.

Must Have Qualifications:

  • 18 Years of age
  • Highschool Diploma or GED equivalent
  • Valid NYS Drivers’ License
  • 1-3 years experience working with individuals with SUD disorders and or SMI
  • Education may be substituted for experience. Each year of education is equivalent to 1 year of work experience.
  • Willing to participate in peer certification training, acquiring CRPA-P no later than 4 months after employment.

Nice to Have Qualifications :

  • Associates Degree in Behavioral Health
  • Bachelors Degree
  • Certified Peer Support Specialist (NYCPS) trained by OMH or OASAS
  • Certified Peer Advocate (CRPA)

Duties:

1. Serves as an active member of the client’s treatment team, including participation in team meetings and treatment planning meetings. Team members include but aren’t limited to the care managers/HARP coordinators.

2. Manage individual caseloads, including referrals, intake, prior authorization with insurance providers, input/management of service thresholds, development of individual service plans and care plan reviews/updates as well as discharge/transition planning. Will additionally complete/coordinate care with referral source for crisis plan.

3. Provides services as approved by HARP and the MCO as described in the BH HCBS Provider Manual.

4. Serves as role model for clients by sharing own recovery process and teaching effective coping techniques and self-help strategies depending on HCBS service line.

5. Provides case management, resourcing, service collaboration for individual clients as extension of case management services.

6. Provides advocacy services on behalf of clients.

7. Informs supervisor, and other members of treatment team as indicated, of issues/concerns related to individual clients.

8. Participates in required trainings to maintain certification and HCBS requirements.

9. Knowledge of Crisis Intervention and Early Engagement

10. Helps facilitate outreach and encourage engagement in appropriate treatments

11. Provides services in the community which may include client’s homes.

12. Providers transportation services as appropriate.

13. Other duties as assigned.

Other Skills/Knowledge and Experience

1. Good interpersonal skills and written and oral communication skills.

2. Knowledge of applicable population group and principles/philosophy of recovery.

3. Ability to multi-task, have good problem solving and time management skills and the ability to remain calm in a crisis

4. Strong networking skills

5. Required to participate in an on-call rotation.

6. Must have NY state driver license.

Job Type: Full-time

Pay: $16.50 - $18.00 per hour

Expected hours: 37.5 per week
