Job Summary:
The Care Navigator, Social Worker will provide basic information and process referrals in addition to direct, hands-on coordination assistance. They will be responsible for conducting supportive service needs screening, referrals; identifying, accessing, and coordinating services; monitoring receipt and follow through of services, and building partnership and communicating with service partners. The function of the enhanced service coordinators will assure that elderly residents obtain necessary supportive services from the community so that they are able to age in place.
Minimum Qualifications:
Bachelor’s degree and 2 years’ experience in Health or Human Services and knowledge and experience with elderly, community resources, housing, employment and other professional and nonprofessional services.
Duties:
1. Outreach, enrollment and initial supportive services needs screening
2. Partner with coordinator to assess residents service needs of new residents within 14 days of move in.
3. Complete annual needs reviews that are required for all residents in the program.
4. Provide general care navigation work including accessing financial support for personal assistance, homemaker services, meals-on-wheels/congregate meal provision, transportation, counseling, visiting nurse, preventive health screening/wellness services and legal advocacy
5. Establish linkages with agencies and a develop efficient workflow processes.
6. Communicate and meet periodically with service provider to discuss and share information concerning residents and identify ways to assess residents
7. Provide the necessary link between management and the residents
8. Problem solve to assess service needs and help determine eligibility for supportive services
Other Skills/Knowledge and Experience:
1. Demonstrates ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.
2. Knowledge of local behavioral health services and substance abuse agencies.
3. Ability to multi-task in a fast-paced environment, have good problem-solving skills, as well as excellent time management and organizational skills and the ability to remain calm in a crisis while providing crisis intervention.
4. Must be able to type 30 WPM.
5. Proficient in Internet navigation, Microsoft Office, Outlook, Word, and Excel.
6. Proficient in the use of electronic health records.
