We are looking for a dynamic Registered Nurse (RN) to join our phenomenal team of clinicians!

The Registered Nurse will administer skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the guidance and supervision of the Director of Clinical Services/Nursing Supervisor.

RESPONSIBILITIES:

• Provides services in accordance with the plan of care that approved by the following physician.
• Makes the initial evaluation visit for nursing and regularly re-evaluates the patient’s nursing needs according to the plan of care and Agency defined time frames.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Coordinates services with other clinicians within the disciplinary team.
• Processes orders and notifies physician of patient needs and changes in condition.
• Educate the family/caregivers on how to assist in caring for the patient medical needs.
• Conducts patient care conferences on patients assigned to his/her care.
• Participates in peer review and performance improvement as assigned.
• Participates in utilization review of medical records as assigned.

QUALIFICATIONS:

• Graduated from an accredited two year college or for year college with an Associates and/or bachelor's in nursing, required.
• Active license in the state(s) to which you practice and/or compact state license, required.
• Two (2) years nursing experience working in an acute care/med surge environment, required.
• One (1) year home health experience, preferred.
• Ability to exercise initiative and independent judgment.
