Fishkill Home Careis seeking a caring, dedicated physical therapist to provide quality care to its Patients. The Physical Therapist Supervisor evaluates and treats patients/Patients, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

WE are Paying a $2,500 signing bonus for this position (to be paid upon successful completion of 90 day probationary period)

PT Duties Include:

• Supervise the activities of physical therapy assistants.
• Evaluate Patients conditions & develop appropriate physical rehabilitation plans.
• Help Patients improve their mobility.
• Relieve PatientsResident’s pain using exercises, hands-on therapy & stretching.
• Teach Patients how to use physical therapy equipment.
• Help Patients through their tasks and monitor their performance.
• Maintain Patient confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Supervisors PTA according to State and Agency Policy.
• Submit all billing logs and necessary information on a timely basis.

PT Requirements Include:

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Nursing Home Experience preferred
