firsthand is changing the way individuals living with Serious Mental Illness (SMI) get care – by putting them at the center of everything we do. Our peer led model aims to build trust with these individuals, many of whom have been underserved, stigmatized, deprioritized and have very often lost trust in the systems intended to serve them. Our holistic approach includes partnering with our team of peer recovery specialists, licensed social workers and nurse practitioners, and focuses on meeting them where they are and walking with them side by side, as a trusted guide, partner, and first call, on their journeys’ to wellness.

As Clinical Director you will:

• Oversee community-based Health Guides (APPs) throughout Tennessee, including reviewing charts and meeting 1:1 with each direct report on a weekly basis
• Partner with the Tennessee Senior Director, Community Operations (SDCO) to lead, motivate, and inspire community teams of peers, social workers, and clinicians deeply dedicated to firsthand’s mission; regular visits to existing teams expected to provide in-person guidance
• Communicate and coordinate with other community team leaders regarding Health Guide services and collaboration
• Develop and maintain partnerships with hospitals and outpatient care providers to support outreach to find unengaged patients, referral, coordination of care and disease management and collaboration with firsthand health guides and community teams
• Work closely with firsthand's Support team, in especially close partnership with the VP, Clinical Operations to develop and improve care delivery, clinical quality improvement processes, and outcomes

As Health Guide Flex, you will:

• Be a flexible member of multiple care teams, primarily in Tennessee, moving seamlessly across multiple sites. Perform whole-person clinical assessments, including:
• Gathering information from outside sources (especially other care providers)
• Assessing both behavioral and physical health conditions through a thorough history and physical examination
• Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
• Organizing the information in a manner that supports successful disability applications (if appropriate)
• Conducting visits in the field or in an individual’s home when needed
• Making individualized recommendations and/or referrals for programs/care provided by firsthand or outside providers, based on the assessed need

• Strong management and supervisory skills, including organizing and leading initiatives across multiple teams
• Excellent clinical acumen in outpatient chronic disease and mental health care

• Comfortable with conducting home/ community visits and doing assessments in the community
• Comfort working as part of a multidisciplinary team, and enjoy mixed roles and duties

• Desire to implement and deliver data-driven care that is integrated across chronic medical conditions, behavioral health conditions, inpatient settings, and outpatient settings
• Adapt rapidly to variations in team culture and process
• Ability to rapidly develop new professional working relationships with colleagues and patientsFlexibility, and the willingness/ability to travel on a regular basis

The experience you bring to this role includes:

• Certified APN or PA in primary care
• 5 years clinical care in a primary care setting
• Prior clinical operational leadership experience, including directly supervising clinicians
• Working with and managing a population with challenges such as behavioral health and/or SUD conditions
• Developing and delivering educational programs to nurses and affiliated medical staff
