firsthand is changing the way individuals living with Serious Mental Illness (SMI) get care – by putting them at the center of everything we do. Our peer led model aims to build trust with these individuals, many of whom have been underserved, stigmatized, deprioritized and have very often lost trust in the systems intended to serve them. Our holistic approach includes partnering with our team of peer recovery specialists, licensed social workers and nurse practitioners, and focuses on meeting them where they are and walking with them side by side, as a trusted guide, partner, and first call, on their journeys’ to wellness.

**This role can be based in any of our Tennessee offices: (Kingsport, Knoxville, Johnston City, Memphis or Nashville). Also note that regardless of “home” office, the person hired to this role will travel across all Tennessee offices (25-50% of the time).
In each of firsthand’s Tennessee markets, a Health Guide (licensed Nurse Practitioner or Physician Assistant) leads the medical aspects of team-based care for the individuals that firsthand serves. The Health Guide is responsible for whole person assessments and management of our vulnerable population, collaborating closely with external primary care and behavioral health providers, as well as an internal team of firsthandGuides (peers) and Community Resources Guides (social workers).
Health Guides provide comprehensive assessment, care coordination, disease management, and transitional care to individuals who are either minimally engaged or not engaged at all with ongoing primary and behavioral health care. Health guides do not provide ongoing full-service primary care or behavioral healthcare.
As Clinical Director, you will spend approximately 70% of your time providing oversight to 8-16 APPs across multiple sites in TN, and approximately 30% of your time providing direct patient care as a Health Guide Flex. The Health Guide Flex is a licensed NP or PA who fills in for or augments the market-specific Health Guides when they experience a vacancy or during periods of high demand, through a combination of travel and telehealth.
The Clinical Director will also be in collaborative practice and receive clinical supervision with the Chief Medical Officer (CMO) of firsthand, a board-certified psychiatrist, and a physician collaborative practice supervisor. The Clinical Director reports to the VP, Clinical Operations, and works closely with the CMO, state community team leaders and corporate level support team members to manage and improve performance and the quality of care provided by health guides.
