First Resources of Ottumwa is searching for a dedicated Full-time Service Coordinator.

Acts as central point of contact for Hope House Treatment and Recovery Services. Is supervised by Clinical Supervisor or designee. Responsible for supervision of Residential Technicians to ensure services are provided 24 hours, 7days a week.

Implements monthly staffing, with training and ensures agency requirements are communicated and met. Is responsible for assisting clients in meeting state mandated requirements, admittance and discharge planning. Provides case management for C.I.N.A cases.

Essential Job Functions:

• Coordinate court, DHS, meetings of clients and provide reports relative to case management as needed.
• Act as liaison with the Department of Human Service relative to case management needs.
• Monitor and direct or provide transport for clients as needed to meet appointments.
• Assure new admits have substance abuse, mental health screenings as directed by funding source and credentialing.
• Fostering and maintaining positive professional relationships with community stakeholders.
• Monitor and staff scheduling to assure optimal case coverage.
• Serve on partnering committees as requested.
• Maintain and place food orders for the facility and provide required reporting Food Bank monthly.
• Facilitate Life Skills Group as needed.
• Work collaboratively with CADC to review client's weekly itinerary. Responds to emails within 24 hours and phone calls within 2 hours.
• Provides direct supervision of Residential Technician and weekly supervisory sessions with staff to ensure quality services are being provided. Complete supervisory review documentation
• Participates in the on-call rotation of a designated group. May be required to work direct care hours if coverage is not found during on-call period.
• Fill in hours as needed and be willing to work a flexible schedule.
• Responsible to maintain the highest level of confidentiality always.
• Must maintain a professional appearance and work effectively with people.
• Performs other related duties as required to accomplish the objectives of the position, as assigned.
• Ability to be flexible as the job may require late working hours outside of the normal business hours

Requirements:

• Prefers a candidate with a least an Associate's Degree in Human Services or related field. Otherwise a high school diploma with experience will be considered.
• At least two (2) years' experience working with basic knowledge of Substance Abuse disorders is preferred. Demonstrated ability to interact positively with others and have the desire to work with persons with substance abuse disorders.
• Prior knowledge of mental health or waiver services.
• Ability to embrace rapid change, meet multiple demands simultaneously and efficiently with little supervision
• Prior supervisory experience preferred.
• Must posses basic knowledge of computers and computer based programs.
• Able to lift a minimum of 40 pounds without restrictions.
• Successfully complete the new hire orientation checklist.
• Satisfactory attend all required trainings by agency.
• Successfully complete the established medication administration training and pass medications accordingly.
• Must be able to obtain CPR/First Aid Certification within the initial 30 days of employment, and actively maintain certification throughout employment.
