Company DescriptionFirst Healthcare Networks, LLC We are dedicated to the highest level of professional care to return our patients to optimal health, to enhance their quality of life and meet their social and emotional needs.We are a local Home Health agency clinician owned and operated.Job DescriptionGeneral Purpose:To provide personal care and incidental/instrumental activities of dailyliving as assigned to patients/clients in the home setting as permittedby the policies and procedures of Interim HealthCare® and any otherfederal, state and local law or regulation.Essential Functions:Assist, perform and train patient/client and family in activities ofdaily living such as bathing, grooming, toileting and elimination andadequate nutritional intake.Promote safe environment and maintain unobstructed pathways while providing care.Assist with ambulation, transfers and/or range of motion exercises.Assist with incidental/instrumental activities of daily living such asshopping, meal preparation, socialization activities, homemaking andmaintaining a clean and safe physical environment and medicationreminding.Utilize infection control measures such as universal precautions, hand washing, and personal protective equipment.Recognize, document and report changes in patient/ client condition and safety to supervisor.Organize self to carry out visits/shifts and tasks.Attend mandatory Interim HealthCare inservices and provide requested document to keep employee file current.Completes other assignments as requested and assigned.May have access to personal health information (“PHI”) necessary tofulfill the above duties and responsibilities. Access to use and abilityto disclose PHI is further defined by each organization/department. QualificationsEDUCATION: Must be certified by Department of Public Health as a Certified Nurse's Aide. Ability to speak, read and write English. Ability to follow written and oral directions. TRAINING AND EXPERIENCE: Must participate annually in 12 hours of in-service training to maintain certification. ESSENTIAL JOB FUNCTIONS: Ambulate, position, transfer, and turn residents at a minimum of fifty (50) pounds, frequently. Turn, crouch, kneel, stoop, bend below waist, and reach above shoulder height, frequently. Climb stairs, frequently. Walk, sit and stand frequently. Lift and/or carry objects at a minimum of fifty (50) pounds, frequently. Use sight and auditory senses. Possess good finger and hand dexterity. Additional Information
