Work with HD administration to evaluate changing needs from both top-down and bottom-up perspectives and evolve/evaluate response mechanisms. Specifically, this position will work closely with supervisors to:

• Coordinate with KDHE and other data sources to obtain information necessary for LHD functions, evaluate trends and policy/program mandates at state/federal levels.
• Perform preliminary analysis of various in-house data and compile reports as needed.
• Assist with grant development: compile necessary information through collaboration with other employees, database research, and other factfinding actions, assist with developing and implementing grants, ensure compliance with grant requirements.
• Assist with creation of data-gathering tools and community education/informational materials using valid/appropriate sources as needed.
• Deliver educational sessions/materials to community/staff through different media outlets as needed.
• Assist with organizing meetings/sessions that help perform the above functions, attend meetings/trainings as required.

Conduct disease investigation and contact tracing

• Obtain and review patient demographic, location, laboratory, clinical and risk information to determine investigation priorities per protocols for all reportable diseases.
• Facilitate referrals for testing and medical evaluation for clients.
• Assist with COVID-19 control efforts: inform patients and people about results and isolation protocol as needed, track and update disease/vaccine data, complete documentation/records/tracking processes per HD/State guidelines
• Complete data entry and generate additional reports as required.
• Collaborate and coordinate with a team of public health professionals (local/state) to complete above functions.
• Keep abreast of and follow Departmental and State guidelines for performing above functions

Complete additional support functions

• Participate in meetings, trainings, and activities to continue education and enhance quality of program outcomes.
• Assist in clerical duties including answering phones and scheduling appointments for clinic services as required
• Perform offsite services as required.
• Maintain confidentiality of all information and follow internal HIPAA policies and procedures.
• Respond to change productively and complete all other tasks as assigned.
