Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. With over 200 employees we strive to provide high-quality and innovative health care services to the Finger Lakes Region.

Job Summary:

The Patient Navigator is responsible for facilitating communication and coordinating care among patients, family members, community, and healthcare providers to promote better health outcomes. This position serves as a guide for patients to work through the healthcare process, working closely with provider and nursing teams. The Patient Navigator will also serve interpretation needs for staff as needed to assist our Spanish-speaking population during their time of treatment.

Duties/Responsibilities:

  • Responsible for assisting the navigation of patients through healthcare appointments
  • Assists patients in completing paperwork, such as cancer Services program, financial aid, prescription assistance, intake paperwork etc.
  • Assures that patient flow is smooth, attends daily huddles 
  • Interprets for patients
  • Actively participates in pre-visit 
  • Assists with follow up on patient no shows, 
  • In an effort to reduce no shows and barriers to care, analyses high-risk patients, and addresses barriers to care and Social Determinants of Health 
  • Maintains/works on registries as indicated per program
  • Assists with scheduling, and rescheduling patient appointments for medical, dental, interpretation etc. 
  • Adds and completes appointments on Resource schedule, and documents patient encounters in the electronic health record.
  • Promptly responds to all referrals, telephone encounters, and actions assigned by a healthcare provider, according to FLCH policy.
  • Maintains program brochures and community resources inventory, responsible for updating resources when necessary. 
  • Maintains a tele-dental caseload, responsible for following families and adhering to workflow protocol
  • Responsible for empowering patients by providing current information and education
  • Refers patients to appropriate agencies as necessary.
  • Advocates for patients, helping them access resources and services as needed
  • Performs other duties as assigned by supervisor

Required Skills/Abilities: 

  • Must be able to fluently speak in both English and Spanish languages
  • Experience with interpretation or interpretation certification preferred
  • Possesses basic computer skills, including Microsoft Office and the ability to learn new software
  • Possesses strong organizational skills
  • Possess the ability to prioritize
  • Possesses the ability to work effectively with culturally diverse populations

Education and Experience:

  • High School or Equivalency Diploma
  • Minimum of 2 years of experience in a similar role
  • Possesses New York State driver’s license
  • Current New York State automobile insurance in compliance with FLCH underwriting agreement

Physical Requirements: 

  • Must be able to hear and communicate with clients and staff via phone, web media, and in person.
  • Must be able to lift up to 30 lbs
  • Must be able to sit for extended periods of time
  • Must be able to kneel, crawl, or bend into awkward spaces
