Health Care Coordinator (similar to a Director of Nursing)

Locations: Group Homes in Bryan and West Unity, OH

Filling Homes needs a Registered Nurse (RN or LPN) to serve as a Health Care Coordinator (similar to a Director of nursing). The Health Care Coordinator is responsible for the health care of residents in two, six-bed group homes for individuals with intellectual and developmental disabilities. This individual must be willing to work as part of a caring community of respect, dignity and partnership with individuals served and their guardians and team members.

The Health Care Coordinator is responsible for both administrative and clinical functions and will need to learn the federal and state regulations that govern the homes.

What you will do while working at Filling Home:

  • Actively monitor and oversee health care services for the residents.
  • Initiate and review the nursing physical assessments of the residents on at least a quarterly basis.
  • Actively participate as a team member in the development of the Individual Habilitation Plan (IHP), including the development of health teaching programs.
  • Oversee and delegate nursing for staff members, monitoring medication passes and skills.
  • Communicate and consult with physicians, and attend some appointments with residents.
  • Participate in compliance surveys annually.
  • Responsible for weekend consultation and on-call duties on a rotating basis.

We are looking for:

  • A Registered Nurse preferred "experienced Licensed Practical Nurse will be considered", must hold an active, unencumbered State of Ohio nursing license
  • Prefer 2 years of experience in a health care setting
  • Experience with individuals with intellectual and developmental disabilities preferred
  • Possess strong communication, leadership, and organizational skills
