• The Health Center provides both family practice and pediatric care to area residents, regardless of ability to pay. For patient convenience there is an on site laboratory, pharmacy assistance program, social workers and a care navigator. All fees are based on a sliding scale, and most insurances are accepted including Ohio Medicaid and self-pay. Financial assistance available for those who qualify. Fees are based on ability to pay.

Compensation and Benefits:

• Competitive salary range based on experience and training (to be discussed at interview). Provider may submit salary expectations with presentation.

Required:

• Board Certified NP/PA
• OH license (or willing to obtain)
• Three years' outpatient Family Medicine experience
• Full-time availability

Pay: $99,548.26 - $115,000.00 per year
