FGC’s adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.

1st $1,000.00: after 30 days of employment

2nd $1,000.00: after 6 months of employment

Essential Duties:

• Complete FGC intake screening documents via face-to-face or telehealth to determine appropriate level of care
• Respond to bookings/emails for daily assessment assignments in a timely manner
• Administer the biopsychosocial assessments in EHR (SAMMS) with attention to clinical detail and justification from DSM
• Complete all intake paperwork including admission notes within one business day of the intake
• Utilize the SOAP clinical note format for documentation
• Scan all admission documents into SAMMS within one business days of assessment
• Coordinate admission process with the Retention Specialist to ensure successful completion of all steps by patient
• Communicate to appropriate staff/referral sources as needed
• Report to appropriate staff immediate need for external referrals
• Ensures timely review of all chart audits and correction returns for Performance Improvement (ARCS). Corrections submitted by identified due date.
• Receive Naloxone (NARCAN) training on recognizing the signs of an opioid overdose and how to administer the opioid overdose reversal drug
• Provide Naloxone and training to all in takes
• Utilize Outlook/TEAMS for weekly staff meeting participations
• Attends regular team and agency meetings and trainings
• Demonstrates flexibility in a fast-paced environment and willing to take initiative to assist other team members
• Performs other duties as assigned

Schedule: Monday - Friday (7:00a - 3:30p)

Pay Rate: Up to $20.00 per hour; based on experience

Education and/or Experience: Associate degree (or higher) preferred. Two years' experience in out-patient behavioral health preferred. Individual must posess good communication skills with basic knowledge of Microsoft Word and Excel. Knowledge of substance abuse pathology preferred. Some working knowledge of ASAM clinical guidelines, DHS/SUPR contractual language and deliverable expectations, JCAHO/CARF guidelines, DEA, and CSAT rules, regulations and guidelines is preferred.

Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC)  required or able to obtain within allotted time from hire date
