Ferry County Health is Hiring a LPN Long Term Care Days ($5000.00 Sign on bonus) Near Republic, WA

JOB SUMMARY

Assesses, plans and implements the nursing care of residents within the patient care unit settings with the supervision of a Registered Nurse. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by NAC’s and supportive staff members. Participates in resident and family teaching as determined in scope of license. Maintains role as patient advocate with a focus on the district’s mission.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assesses extended care residents on admission and on a weekly basis as well as when conditions change. Documents appropriately including ongoing assessment and task completion in the electronic health records, under the supervision of a Registered Nurse.
  • Assesses the resident’s condition and nursing needs, prescribes appropriate nursing action.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.
  • Documents changes in residents' condition and nursing care on progress notes and care plan.
  • Ability to interpret results of waived tests; take appropriate action on waived tests results.
  • Administers medications and follows the medication rights and reduces the potential for errors.
  • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
  • Supports and maintains a culture of safety and quality.
  • Meets current documentation standards and policies.
  • Learns the location of all equipment in order to best collaborate assistance when needed during emergent situations.
  • Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.
  • Notes and carries out physician’s orders in a timely manner. Reports any scope of practice concerns to the charge nurse immediately.
  • Gives a thorough report to oncoming shift by participating in end of shift report process.
  • Follows through as needed on information given by shift report, resident or family regarding resident concerns.
  • Implements plan of care of the resident based on assessments and goals established by the interdisciplinary care team.
  • Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
  • Supervises other members of the healthcare team as needed to accomplish the plan of care which has been developed.
  • Demonstrates competence in technical and manual skills according to scope of practice for LPNs/LVNs.
  • Functions as a resident advocate by protecting the resident’s rights.
  • Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
  • Interacts with residents, families, co-workers, managers and other departmental staff in a kind, knowledgeable and professional manner.
  • Consistently involves resident and family in planning for care, treatment, teaching needs and discharge plans.
  • Participates as needed in resident care conferences and updates plan of care, as appropriate.
  • Functions as a leader for the NAC’s; delegates in a manner that contributes to the excellence of the nursing unit to which LPN/LVN is assigned.
  • Participates in orientation of new staff after preceptor training. Demonstrates ability to remain calm in emergency and/or stressful situations.
  • Consistently follows the policies and procedures of the facility.
  • Exhibits genuine interest in and appreciation of adults in later years of life.
  • Perform related duties as assigned by management.

PROFESSIONAL REQUIREMENTS

  • Actively checks and responds to emails throughout the day.
  • Completes all mandatory education within given time-frame.
  • Maintains regulatory requirements.
  • Reports to work on time and as scheduled.
  • Wears identification while on duty; uses computerized punch time system correctly.
  • Represents the organization in a positive and professional manner in the community.
  • Attends annual review and department in-services as scheduled.
  • Attends at least 75% of staff meetings; reviews all monthly staff meeting minutes.
  • Actively participates in the department’s quality improvement (QI) activities.
  • Complies with all organizational policies regarding ethical business practices and the employee handbook.
  • Communicates the mission, vision and values of the organization as well as the focus of the department.
  • Meets dress code standards for the department; appearance is neat and clean.
  • All Ferry County Health employees are required to maintain confidentiality in accordance with organizational policy, state and federal regulations, including but not limited to, the Health Insurance Portability and Accountability Act (HIPAA).

JOB KNOWLEDGE & QUALIFICATIONS

Education and/or Work Experience Requirements

  • Graduate of an accredited LPN/LVN program.
  • Current Licensed Practical Nurse/Licensed Vocational Nurse in State of Washington.
  • Current AHA BLS certification.
  • Active ACLS and PALS certification, or agreeable to take the district led classes within six months of hire.
  • Current certification in IV therapy, or agreeable to participate in the district IV competency training program with the Nurse Manager within 6 months of hire.

OTHER SKILLS & ABILITIES

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the general public.

Mathematical Skills

Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical solutions.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
