Program Description

The goal of the Crisis Antioch Response Program is to provide 24-hour alternative response services to low-level 911 calls from Antioch Residents. The AQCRT non-police personnel can assist with crisis intervention and connect individuals to community-based services/resources. Resident involvement with this program is voluntary and the team's response will focus on de-escalation, mitigation, and prevention of repeated non-life-threatening emergency situations. Implementation of the AQCRT Program will build a collaborative network of service providers in the City of Antioch, provide a safer response to behavioral health, quality of life and low threshold medical calls and allow law enforcement personnel to focus their efforts on major emergencies and investigative efforts. Data collection activities from the AQCRT Program will support opportunities for evaluation and inform strategies for improvement.

Position Summary

Under the direct supervision of the AQCRT Program Director, Case Manager will provide low-level case management support to a broad range of low-level calls and situations without police or EMS personnel. The Case Manager, when appropriate will provide on-site crisis assessment, de-escalation, and conflict mediation and adhere to the principles of Motivational Interviewing, Harm Reduction, and Trauma Informed Care. The Case Manager will support linkages that can address residents goals, including engaging in mental health, substance use treatment and employment via facilitating the linkage to a AQCRT Behavioral Health Specialist and/or another provider when appropriate. The Case Manager will act as a liaison with community partners and other social service agencies and adhere to laws regarding confidentiality and reporting requirements while also conducting, (at times) community outreach and may provide transport to necessary service locations so that engaged Antioch resident can begin a relationship with a provider immediately. The position requires strong interpersonal skills to make connections and build relationships with community residents in need.

AQCRT Case Manager Responsibilities

  • Works a flexible, full-time schedule to accommodate program/client needs. Some evening and weekend hours are required. This is not a 9-5 position in an outpatient clinic.
  • Performs intake and comprehensive assessments with adults and TAY who may have (substance use disorders).
  • Provides case management services including evaluations, service plan development, assisting clients with accessing housing, financial, medical, and other needed services. Coordinates with other service providers.
  • Provides individual therapy to stabilize clients and prevent decompensation.
  • Provides crisis prevention planning and crisis intervention and support, as clinically appropriate.
  • Conducts community outreach and engagement in order to safely provide services to clients safely within the community, as needed.
  • Maintains client records after each client contact or service, in accordance with Felton Institutes mandated standards and ACPD requirements.
  • Contributes to team meetings and case conferences through presentation of cases, discussion of clinical issues, and exchange of information and ideas.
  • Provide Support for Clinical and Peer staff.
  • Other duties and responsibilities may be assigned as required in order to meet program goals and objectives.

Minimum Qualifications - Case Manager

  • A minimum of 2 years of case management experience with an adult & TAY populationbehavioral health and substance abuse and justice impacted preferred.
  • A minimum of 1 year of experience working with a Justice-involved population preferred.
  • Bachelors degree in social work, Counseling, or Psychology preferred.
  • Bi-lingual/Bi-cultural Spanish-speaking case managers encouraged to apply.
  • Ability to work cooperatively on a multidisciplinary team, and to work with clients of diverse ethnic and cultural backgrounds.
  • Familiarity appropriate treatment interventions preferred.
  • Utilize evidence-informed and evidence-based interventions to address client needs and goals.
  • Ability to meet with community partners acting as current access and intervention points.
  • Meet documentation and other deadlines; comply with requirements for Medi-Cal, when applicable.
  • Perform administrative tasks within timelines (chart completion, meeting attendance, etc.)
  • Connect clients with short and long-term support services such as housing assistance, public transportation vouchers, emergency food gift cards, and personal grooming and hygiene products.
  • Assist clients in applying for public benefits.
  • Ability to develop Individualized Intervention Plan that includes service linkage, as well as intervention and personal change goals.
  • Working knowledge of a variety of computer software applications (i.e., Microsoft Word, Excel, Access, PowerPoint, Adobe Acrobat)
  • Strong organizational and time-management skills
  • Strong communication (both verbal and written) and interpersonal skills.
  • Ability to prioritize projects and follow through.
  • Ability to maintain proper documentation as determined by programmatic requirements.
  • Ability to be a team player with a welcoming attitude.
