Job Description:

The LPN shall provide skilled nursing services to various Home Health patients under the direction of the RN Case Manager or Director of Nursing. The LPN shall follow physician orders and HHA policy and procedure.

Follows physician orders and facility policy and procedure in ordering, storing, administration of drugs, treatments, and tube feedings.

• Being in compliance with the State of AZ scope of Practice and Nurse Practice act.
• Complete mandatory CEUs to maintain LPN licensure.
• Follows specific physician orders for the administration of care as directed on the 485 plan of care.
• Ability to follow philosophy, objectives, policies and procedures that enhance high standards of skilled nursing practices.
• Keeps abreast of new knowledge and information so that clients will be assured of receiving up?to?date methods of care.
• Participates in formal continuing education sessions and in-service sessions.
• Maintains open relationships and communication with clients, clients families, physicians, and other HHA staff involved in the care of the client.
• Reviews incident reports and communicates with DON to assess possible cause.
• Follows all State and Federal rules and regulations as they pertain to the Home Health.
• Is responsible for following Infection Control Manual regulations.
• Responsible for an active role in making sure the rehabilitation program is directed towards assisting each client on their case load to achieve and maintain the highest level of self?care and independence as recorded in the client's care plan.
• Responsible for turning in Nursing visit notes within an 48 hours of visit completion.
• May be assigned other duties and responsibilities by the DON or
• Documents each home health visit within the medical record to include on the nurse's notes significant observations regarding the resident's needs for and/or response to medications/treatment.

LPN Visit: $50/visit

Hourly: $28

• Must be a graduate of an approved school for licensed practical nursing or registered nursing.
• Must hold a current, valid license in the State of Arizona.
• Must have an understanding of DHS rules.
• Previous experience in a geriatric setting preferred.
• Must be flexible and have a desire to communicate and work with seniors.
• Minimum of 2 years' experience.
• Knowledge of Home Health rules and regulations.
• Must be motivated, self-starter, with a passion to serve seniors.
