Fellowship Community is Hiring a Health Unit Coordinator Near Whitehall, PA

3p-11p, includes every other weekend and holiday.

Whitehall, PA

The role of the Health Unit Coordinator is to assist the Unit Manager and the nursing staff / interdisciplinary team in the day-to-day communications and the clerical tasks and organization of the nursing unit. All assigned tasks are carried out in accordance with policy and procedures of Fellowship Manor and the Mission and RISE Core Values of Fellowship Community.

ESSENTIAL QUALIFICATIONS/CREDENTIALS

§ High school education required.

§ Post-secondary education preferred.

§ Proficiency in medical terminology required.

§ Must be able to read, write, comprehend and speak English.

§ Intermediate/advanced computer skills (Email, Microsoft Word, MatrixCare) required.

§ Must demonstrate effective communication and people skills.

§ Must be certified in Basic Life Support.

§ Must understand geriatrics, dementia short-term and long-term care.

§ Clerical experience is preferred.

§ Ability to prioritize, organize and address demanding needs as they arise.

§ Must possess positive customer service skills to address internal and external customers.

§ Work performance must reflect the essential elements of Fellowship Community’s Core Values.

§ Must successfully complete the new hire Fellowship Community orientation.

§ Meets the requirements of the pre-hire assessment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Maintain active medical records in a neat organized fashion (includes filing).

§ Thin active medical records as directed.

§ Uses MatrixCare software competently.

§ Order and maintain adequate levels of supplies for nursing units.

§ Record medical and administrative information in accordance with established charting and documentation, policies and procedures.

§ Maintain up-to-date resident census for assigned unit.

§ Copying/ scanning of documents as assigned.

§ E-mail and fax communications as necessary.

§ Answer telephone, page calls, deliver messages to residents, employees,

etc., as necessary.

§ Maintain a current listing of up-to-date phone numbers.

§ Record appropriate resident identification data on designated medical

records, MatrixCare, wristbands, and picture ID for emergency box and MAR.

§ Record vital signs, I & O’s and weights daily.

§ Forward diet changes to the dietary department.

§ Keep nursing services personnel informed of resident’s appointments.

§ Assist in arranging and making resident appointments for diagnostic

and therapeutic services and processing the paperwork.

§ Arrange for Fellowship Manor personnel to transport residents as necessary.

§ Arrange/schedule transports as necessary outside the facility.

§ Assist with all admissions, transfers and discharges as directed by the

Clinical Care Manager /Case Manager.

§ Assist in scheduling of personnel and staffing as directed by Staffing Coordinator/Clinical Care Manager/Supervisor.

§ Inform nursing personnel of new admissions, expected time of arrival

and room assignments. Prepare chart as per protocol and assure room is ready.

§ Restock the form drawers as necessary.

  • Keep nurse stations organized, neat and stocked in a uniform manner.
  • Provides educational information to the resident’s family as appropriate.

§ Greet newly admitted resident/family members and escort to assigned room.

§ Inform family members of resident needs, occurrences, appointments, etc.,

as directed by Nurse Manager/Team Leader.

§ Assist in transport of residents to and from activities as required.

§ Assist in orientation of new employees as necessary.

§ Treat all residents with dignity and respect.

§ Report all instances or allegations of resident abuse.

§ Keep resident information in the emergency box up-to-date.

§ Assist in the MDS process as directed.

§ Assist Medical Records in the closing of a resident chart.

§ Support a restraint-free environment and resident safety.
