Federation of Organizations is Hiring a Licensed Practical Nurse (LPN) Near Brentwood, NY

Federation of Organizations is looking for a full-time Licensed Practical Nurse (LPN) to join our Residential Department. the LPN will provides health services including medication management as a member of a multidisciplinary residential team to people with psychiatric disabilities in a transitional licensed residential program.The world does not run out of people to help…and we need people like YOU to help us make a positive change in the community!Qualifications– Currently licensed as LPN by the New York State Education Department.
– Valid and clean NY State driver license.
– Minimum of 1 year of experience in a treatment or rehabilitation setting.
– Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Core & Essential Responsibility
  • Provides management and administration of medications, including supervision of medication, maintaining supply of medications, counting of medication and ordering as well as health office organization on a daily basis.
  • Interface and follow-up with appropriate medical and psychiatric treatment providers within the community and throughout the agency as often as necessary each week.
  • Update EHR within 24 hours of all medication changes.
  • Conduct individual services and groups related to health and wellness as often as necessary each week.
  • Document all interactions in EHR as per agency and program requirements daily.
  • Provide training on medication management and follow medication policy and procedure.
  • Direct supervision and training of Health Office staff including health office activities and medication management services at least twice per month.
  • Interface with medical and psychiatric treatment providers as often as necessary each week.
  • Assist in crisis management as the need arises daily, report all incidents to Program Supervisor and documents incidents as per agency policies/procedures.
  • Provides redevelopment of daily living skills including symptom management, ADLs, medication maintenance, socialization and advocacy skills to the residents daily.
  • Supervise residents to ensure that all living spaces are kept clean and orderly each week.
  • Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.
  • Utilize all agency-based IT applications in a proficient manner including collaborative documentation, review of emails at least twice per shift (at start and prior to close of the shift), as well as utilizing Outlook Calendar on a daily basis.
SALARY RANGE: $55,000 Annually
