Federation of Organizations is looking for a full-time Community Residential Counselor to join our Residential Transitional Services team. This individual will provide supportive counseling and assist with integration into the community for individuals with psychiatric disabilities in a scattered site housing program.

QUALIFICATIONS:

· High School Diploma/GED

· Two years of experience working with individuals with psychiatric disabilities (preferred)

· Valid and clean NY State driver license

· Fingerprinting, criminal record check, approval from NYS Office of Mental Health

CORE & ESSENTIAL RESPONSIBILITY:

· Meet with any clients the Community Integration Specialist assessed to have needs in regards to community integration, benefits and transportation needs as needed.

· Assist Community Integration Specialist with development and maintenance of Tracking Reports on current benefits, when renewals are due and other community services needed.

· Completion of documentation for any contacts made in the EHR within 48 hours of occurrence.

· Completion of Discharge Summary within one week of client’s discharge and any necessary follow up.

· Provide linkage to other services in the community as needed.

· Assist clients in procuring financial entitlements, including educating clients on how to keep track of renewals and changes to their benefits as needed.

· Provide travel training and assist with finding linkages to their appointments and into their community as needed.

· Research events in the community and coordinate trips with clients monthly.

· Actively participate in weekly staff meetings and monthly high risk meetings.

· Complete all required trainings monthly as per the agency-wide training schedule.

· Participate in supervision a minimum of once a month with the Community Integration Specialist and once a month with the Program Supervisor.

· Management of crisis situations under the direction of the Program Supervisor and document incidents as per agency policies/procedures..

· Perform all other job duties as assigned.

SALARY: $19.95/hour
