**Job Summary:**
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide comprehensive patient care, coordinate services, and collaborate with healthcare professionals to ensure the best possible outcomes for patients.
**Responsibilities:**
- Manage and coordinate patient care throughout the healthcare continuum
- Conduct assessments and develop individualized care plans
- Collaborate with physicians, specialists, and other healthcare professionals to optimize patient care
- Provide patient education and support to promote self-management
- Monitor patient progress and outcomes, adjusting care plans as needed
- Ensure compliance with HIPAA regulations and maintain accurate medical records
**Experience:**
The ideal candidate should possess the following skills and experience:
- Experience in behavioral health, infectious disease care, patient care, managed care, PICU, hospital medicine, acute care, and primary care
- Knowledge of medical records management and documentation
- Strong understanding of HIPAA regulations and patient confidentiality
- Excellent communication and interpersonal skills to effectively interact with patients and healthcare team members
