Farnham Family Services is Hiring a Recovery Peer Advocate Near Oswego, NY

JOB DESCRIPTION:
The Recovery Peer Advocate position provides person centered, strength focused services delivered by an individual credentialed as a Certified Recovery Peer Advocate (CRPA) to work with peers in the office and community-based setting providing recovery peer and HCBS services. The Recovery Peer Advocate will provide peer-to-peer support, advocacy,
resource information and self-help services as part of a multi-disciplinary team to clients with substance abuse problems in accordance with the organization’s policies and procedures. The position is for an individual who has lived experience with, or knowledge of, the needs of those diagnosed with substance abuse and how to assist them with accessing community
resources.


JOB DUTIES AND RESPONSIBILITIES:

  • Provide HCBS Services including assessment, individual and family sessions and associated documentation.
  • Provide linkage to participants to formal recovery supports.
  • Engage with individuals that are considering entering treatment.
  • Engage with peers to attend treatment or other healthcare services.
  • Assist peers develop Recovery Plans
  • Guide peers in self-monitor their progress
  • Model effective coping skills
  • Attend court and other system meetings as a support as needed
  • Support peers in advocating for themselves to obtain effective services.
  • Assures the provision of consistent, non-judgmental, consumer-centered services that are strength-based and develop assets.
  • Educate program participants about various modes of recovery
  • Raise awareness of existing social and other support services
  • Complete data entry and monthly statistics to agency
  • Maintains accurate, up-to-date records of all contacts, collects statistical data
  • Utilize an Electronic Medical Record.
  • Attend Partnering Agency and Peer staff meetings as scheduled.
  • Participate in Supervision Sessions
  • Adheres to all applicable local, state, federal and agency regulations, policies, and procedures
  • Attends training, staff meetings, conferences, and seminars, as scheduled
  • Must be able to work a flexible schedule to meet the needs of the program and needs of individuals served.
  • Must follow program and agency policies, procedures, and regulatory requirements
  • Must have the ability to work well with others, be a positive role model and a good representative of the Agency
  • Must model appropriate problem resolution and living skills
  • Must be able to work independently as well as an intricate member of a team
  • Must maintain a high degree of professionalism, good judgment in accordance with CRPA Code of Conduct
  • Must have acceptable physical and mental health to carry out the responsibilities of the position
  • All job responsibilities with be in alignment with value- based health/payment. It is the responsibility of this position to know and understand the specific implications of VBH/VBP on day-to-day duties on the job.

MINIMUM QUALIFICATIONS:
NOTE: Farnham Family Services Training Institute offers the 46 hours of Required training to obtain CRPA-P

  • High School Diploma or GED preferred.
  • Experience as a peer or family member with personal experiences of recovery regarding mental illness and substance use conditions.
  • Have or in process of obtaining Certified Peer Recovery Advocate (CPRA-P) certification at the time of employment or obtain provisional within 3 months of commencement of employment.
  • Prior work experience in peer support and HCBS activities preferred.
